Description

This is a solicitation notice for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6. Solicitation number Request for Quotation (RFQ) 673-3-02 is assigned and shall be referenced on any request for solicitation. Solicitation shall be issued on or about July 20, 2001. The James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, is seeking complete maintenance and repair services at the Orlando VA Healthcare Center, 5201 Raymond Street, Orlando, FL 32803. The contract period is October 1, 2001 through September 30, 2002, with two (2) one year options for renewal. Contractor shall provide all labor, materials, parts, tools, travel and equipment necessary to maintain 2 Philips Radiographic and Fluoroscopy units and all related components, 1 Philips Radiographic and Tomographic unit and all related components, 1 Philips Mammo Diagnost 3000, 1 Philips Medical CT Scanner SK7000, 1 Philips Radiographic Compact Diagnost Medio 50CP, and 1 Medrad Angiographic Injector Envision. Successful offeror shall be able to obtain any manufacturer-specific software and/or parts for total maintenance of each model of equipment described. Successful offeror shall have manufacturer-trained repair personnel. The North American Industry Classification System Code is 811219. The Small Business Size Standard is $5.0 million. All requests for solicitation shall be in writing or via fax at 813-903-4838. The Government shall not be held responsible for the quality of faxes. Pending contract is subject tot he availability of funds.
