Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information provided in this notice. THIS NOTICE IS THE SOLICITATION DOCUMENT. An offer is being requested and a written solicitation will not be issued. Solicitation F22600-01-T-0648 is an unrestricted solicitation. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 97-27. Keesler AFB intends to issue a firm-fixed price contract for a base period of 2 months (1 Aug-30 Sep 01) and a one year option period (1 Oct 01-30 Sep 02) for nonpersonal services to provide insulin pump education and therapy to pre-natal beneficiaries. The Director of the Keesler Center for Maternal-Fetal Medicine will place calls to order Insulin Pump Therapy under the terms of this contract for patients who have had insufficient diabetes control after initial standard insulin injection therapy. The Contractor shall respond within 24 hours of notification by authorized Center personnel and ensure qualified, certified providers perform services within a 200-mile catch area of Keesler Medical Center, Keesler Air Force Base, Mississippi, including Louisiana, Alabama, and Florida. The Contractor shall provide a mini-med, or equivalent, infusion pump for diabetes therapy, education for the patient, 24-hour telephonic contact with the patient for infusion therapy guidance, and frequent provider-initiated personal contact for blood sugar assessment. The Contractor shall submit a written summary on a weekly basis, detailing the infusion pump therapy care provided for the previous week.=20 The North American Industry Classification Code is 621610 and the small business standard is $10 million. This is a DO rated order with a priority identification symbol of C9E. The following Federal Acquisition Regulations (FAR) provisions and clauses apply: FAR 52.212-1 Instructions to Offerors-Commercial Items is amended to read: submit signed and dated offer on Company Letterhead to 81 CONS/LGCB , 310 M Street, Room 102, Keesler AFB MS 39534-2147 at or before 1530(3:30pm) CST 23 Jul 01. FAR 52.212-4 Contract Terms and Conditions-Commercial Items is incorporated by reference with the following addenda: DFARS 252.204-7004 Required Central Contractor Registration. Award will be made based on price, and past performance. Include with quote at least 2 references of past performance to contact. Total Quantity is an estimate since exact price has not been established at the issue of this solicitation.
