Description

The work to be performed consists of furnishing all plant, labor, materials and equipment necessary to remodel the existing ambulance garage area and two existing office spaces into four new exam rooms and three new offices. Additional work shall include the replacement of the existing EIFS fa ade at the main entrance with a new EIFS fa ade and provide a new ambulance access drive adjacent to the emergency entrance. The Plans and specifications use International System of Units (SI) metric equivalent measurements. Project completion time is estimated to be 120 calendar days after notice to proceed. Estimated construction cost is between $100,000 and $250,000. The proposed contract listed herein is set aside for 51% Indian owned, controlled and operated firms under the authority of the Buy-Indian Act. In accordance with the Public Health Service Acquisition Regulation Paragraph 380.503(e) not more than 50% of the work to be performed under a prime contract shall be subcontracted to other than Indian firms. For this purpose, work to be performed does not include the provision of materials, supplies or equipment. Bid documents will be available at no charge on or about July 30, 2001.
