Description

The Federal Correctional Institution, Oxford, Wisconsin is soliciting to establish a non-personal service agreement to provide the services of an Orthopedic Surgeon. The Contractor will conduct minor orthopedic procedures in-house, i.e. cast, reduce fractures, splint, and fit and/or measure patients for orthopedic prosthetics. The Contractor will provide written and/or dictated reports on all patients referred for evaluation and will include recommendations for further diagnostic testing and/or treatment, when indicated. The Contractor will help monitor the attendance of inmate patients who are on call out for patient care, and shall report to the Health Services Administrator any infractions of institution rules/regulations on the part of the inmates. The Contractor shall furnish copies of all state licenses, board certifications, continuing education received during the contract period, proof of professional liability insurance, and a copy of diploma(s) for credentialing pursuant to JCAHO standards and Bureau of Prisons policies. Contractor will provide two, two hour sessions per month, for an estimated total amount of 12 sessions for the expected contract period of October 1, 2001 through September 30, 2002. A firm-fixed option year contract is contemplated, cost will be based on the total price for the basic requirement together with any option(s) exercised at the time of award. All responsible sources may submit a quotation which will be considered by this office. The estimated range of this agreement is $25,000 to $100,000.
