- Description
- The Federal Bureau of Prisons intends to award a Non Personal Services contract for an Optometrist. Duties are to be performed in the Health Service Units, at the Federal Correctional Low & Medium Institutions, Petersburg, Virginia. The services include examination, diagnoses, and making of recommendations to the Clinical Director for appropriate optometric care/furth treatment of ocular disease(s), prescribe eyewear, and any medically indicated features of eyewear for inmate patient's inique ocular needs, perform tonometries and slit lamp examinations when medically indicated/requested, and making necessary adjustments of institution glasses/frames dispensed. The Contractor shall also make recommendations to the Health Services Administrator and Clinical Director for treatment of eye diseases and the procurement of eyewear. The Contractor shall provide written reports of his/her clinical findings and recommendations for treatment. The provider shall provide continuing medical education to the medical staff as required and scheduled by the Health Services Administrator. At a minimum, the Contractor must have a degree from an accredited college or university in the field of Optometry. The Contractor shall carry their own insurance and need to provide a copy of their Certificate of Insurance and Degree and License to Practice with their bid package. This Contract will include a Base Year not to exceed (12)twelve months from date of award, with four (4), one (1) year option periods for renewal. The estimated number of sessions for the Base Year and Option Years is48, (two (2) sessions per month) ( 2 sessions at the Low & Medium Institutions), with one (1) session defined as 4 hours in length.) Solicitation packages will be mailed approximately August 1, 2001, with all bids due on September 4, 2001. It is anticipated this solicitation will result in a Firm-Fixed Price, Requirements type contract. The proposed contract is 100% set aside for small business concerns.
