Description

This is a combined synopsis/solicitation for commercial items prepared in accordancd with the subparts in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number Request for Quotation (RFQ) 610-12-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification System (NAICS) code is 339111, and the small business size standard is 500 employees. The VA Northern Indiana Health Care System (NIHCS) located at 1700 E 38th Street, Marion, Indiana 46953-4589 has a need for the following: 50 ea. Joerns Ultracare Bed, model V770, or equal; 50 ea. traditional laminate bed panels with staff control in footboard, model TRDU770DC, or equal; 50 ea. Joerns turnstyle mattress with perimeter definition, model 550325CC, or equal; 26 ea. Model N431 IV Rod, or equal; and 100 pair (2 pair per bed) Model F17 Horizon side rails, or equal.Quotations shall be submitted on an "all or none" basis. No partial quotations shall be accepted. Offerors shall provide a complete set of provisions, found at FAR 52.212-3, Offeror Representation and Certifications -- Commercial Items, with their proposal. Clauses can be accessed via the internet at www.arnet.gov/far. Provisions and clauses found at 52.212-1, 52.212-4 and 52.212-5 also apply. Quotations shall be submitted to the3 address listed herein and directed to the attention of Marcia Guenin (90C). The solicitation number (RFQ number) shall be referenced on the outside of your response, for identification. The government shall not be held responsible for quality of fax transmittals. Quotes are due July 17, 2001. If offering other than Joerns beds and accessories, please provide descriptive literature so that a complete comparison can be made.
