Description

Provide Nephrology Services for the VAMC, Manchester, NH. Contractor shall provide telephonic consultation onsite upon request of VA physician if he feels their assistance is required. One four-hour clinic shall be held once a month at the VAMC Manchester for the purpose of seeing veterans for nephrology consultations and meeting with VA staff physicians for the purpose of discussing patient issues. The four-hour clinic may be divided into time slots of no less than one-hour blocks (i.e. 9am -- 10am once a week. Consideration will be given to that contractor, unable to establish an onsite clinic at VAMC Manchester, but will, upon the request of a VAMC Manchester physician or his/her designee, schedule an appointment in their office for nephrology consultations. VA patient will be seen within a 30-day period from time of request by the VA physician or sooner if considered emergent. The contractor's site will be within a 25-mile radius of the VA Manchester facility. The term of the contract will be one Base Year, with two Option Years.
