Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1.The request for quotes (RFQ) solicitation number is RMAST-01-235. 2.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-25. 3. This is NOT a small business set aside. The associated NAICS code is 621210 and small business size standard is $5.0 million 4. Dental Hygienist Services on the Trapper Creek Job Corps Center outside Darby, MT in accordance with the attached specifications. SCHEDULE OF ITEMS Quantities shown are estimated and not guaranteed. Payment will be based on actual quantities Base Year -- July 1, 2001 through June 30, 2002: Estimated 50 days service at $_________________per day. BASIS OF AWARD: A PURCHASE ORDER WILL BE ISSUED TO THE FIRM WHO IS FOUND TO BE RESPONSIBLE AND WHOSE QUOTE IS THE LOWEST RECEIVED.
