Description

VA Medical Center, Minneapolis, MN is seeking offerors to provide Community Hospice Care for eligible veterans located in thirty-seven counties of Minnesota; fifteen (15) counties located in Wisconsin and two counties located in Iowa. Offeror must be accredited by a National accreditation organization(s) according to Hospice Standards or Medicare certified and licensed by the state/states in which service is provided and remain in compliance throughout the duration of the contract. VA will conduct the procurement according to a combination of FAR Parts 12, 13 and 15 and under Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. NAICS Code 621999 applies for this procurement and the small business size standard is $7.5 million. Multiple contracts will be awarded for the period of August 1, 2001 through June 30, 2002 with two one-year options to renew, subject to the availability of funds.
