Description

This is an emergency procurement using combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested and a written solicitation will not be issued. All responsible sources are encouraged to submit an offer. In order to be classified as responsive, an offer must provide clear, concise and convincing evidence that the offeror can meet all requirements of the Government. Solicitation number is 502-42-01 and is issued for a Request for Quote (RFQ). Award will be issued using simplified acquisition procedures. This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 97-24 and Veteran Affairs Acquisition Regulation (VAAR) #20. This acquisition is unrestricted. North American Industry Classification System (NAICS) Code for this procurement is 339115 and the size standard is 500 employees. This requirement is for the following: CLIN 0001 -- 1 each non-mydriatic retinal camera, complete with built in TV monitor and power supply, with digital capability to include: Canon CR6 fundus camera, Synemed digital video adapter, Camera eyescape software, capture board, cabling, Pentium computer with 17 inch monitor, mouse pad, surge protector and two medical systems motorized (wheelchair accessible) tables with tops. SK #CR6. CLIN 0002 1 each Polaroid attachment, Canon CRS-PC dual angle Polaroid. The Government will make award to the offeror whose quote conforms to the requirements of the solicitation that best meets the minimum needs of the Government and offers the lowest price. Only the exact name brand and specified part numbers will be accepted. Delivery is required FOB VA Medical Center, Building #136, Alexandria, LA 71306, within 30 days after award. Final inspection will be at destination. Selection for award will be based on past performance and price. Quotes that are non-compliant with any material requirement of this solicitation may be rejected without further consideration for award. This will be a firm fixed price awarded under the provisions outlined under FAR Part 13. The contractor shall extend to the Government the full coverage of commercial sale warranty provided such warranty is available at no additional cost to the Government. This contract incorporates one or more FAR clauses by reference, with the same force and effect as if given in full text. If a change occurs in this requirement, only those offerors that respond to this announcement within the required time frame will be provided any changes/amendments and considered for future discussions and/or award.
