- Description
- Provide healthcare resources in accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor agrees to provide a National Board Certified Occupational Therapist to provide services to the VA Roseburg Healthcare System (VA). Therapist must be able to start on July 16, 2001. If candidate is not available to start on July 16, 2001, offeror will be determined non-responsive, if after award it is determined candidate cannot start on July 16, 2001, contract shall be terminated for cause and award shall be made to next lowest offer. Price is an all-inclusive rate of per diem, travel, and price/cost. The Therapist will be assigned to work in Occupational Therapy, Building #1, from 8:00 a.m. 4:30 p.m. Monday Friday. The Therapist will be required to have knowledge and skill in services provided to an adult population, with the majority of service being provided to a geriatric age group. The Therapist may be assigned to a variety of services to include but not limited to ICU, acute medical/surgical inpatients, subacute care, and skilled nursing and general outpatient. It is preferred that the Therapist have knowledge, skill and experience in providing a variety of hand and UE condition type services to include but not limited to fractures, tendon injuries, neuropathies, trauma, arthritis, soft tissue conditions. Therapist shall perform U/E hand assessment, splinting, standard treatment techniques and management, and education for patients and their family. The Therapist will provide services to patient with a wide variety of acute and chronic medical diagnoses, which may include but are not limited to, CVA, MS, Parkinson, Cancer, LLE amputations, COPD, THR, low back pain, peripheral neuropathy and rheumatic diseases. The Therapist must have knowledge, skill and experience in the provision of standard OT assessment and treatment services with an adult physically disabled population. The Therapist will be required to participate in the interdisciplinary team conferences, family conferences and are required to perform discharge planning. The Therapist may be required to perform home visits for assessment of but not limited to environment, architectural barriers, safety and ADL's. It is preferred that the Therapist have the knowledge and skill in wheelchair and seating assessment as well as prescription writing. The Occupational Therapist must: a. Have an unrestricted license in a US state, territory or b. US commonwealth; b. Speak English proficiently; c. Provide evidence of demonstrated current clinical competence to perform appropriate primary care services; d. Be a US citizen; f. Provide evidence of current professional liability insurance; g. Evidence of current privileges (if applicable); h. Names of most current employer(s) and two peer reference contacts; i. Current copy of BLS certification; j. Be a team player; k. Must be computer literate and type 40 words per minute, as VA Roseburg Healthcare System utilizes a computerized patient medical record; l. Therapist must have at least two (2) years of experience in acute care and/or rehabilitation. NAICS Code 621340/$5 million applies. This procurement has been set-aside 100% for small business.
