- Description
- The Southern Arizona Veteran Affairs Health Care System (SAVAHCS), Attn: Sue Tullier (9-90C), 3601 S. 6th Avenue, Tucson, AZ, 85723, is soliciting for proposals for the Veteran Affairs hospitals in Arizona, (Phoenix, Tucson, &Prescott), and intends to award a contract(s) with options for a total period of 18 months to provide case management to community-dwelling veterans. This contract will assist VA to demonstrate a new clinical model using contract case management for those veterans living in geographically remote areas of Arizona who have primary assignments to the Community Based Out-patient Clinics (CBOC). The purpose is to: maximize the autonomy and continued community residence of veterans; improve access to the continuum of care by enhancing identification of, and referral to, non-VA public and community services; decrease incidence of institutionalization and optimize utilization of out-patient VA services; maintain veteran health-related quality of life and functional status; maintain present per/patient cost of care as roster of veteran participation increases, while enhancing the quality of non-VA services through creative case management strategies; and measure patient-related outcomes related to contract case management. Case management services may vary in scope and frequency, depending on the veteran's intensity of need. The contractor shall provide all labor, supervision and management necessary to carry out the services described in the solicitation. To this end the Contractor shall coordinate health care services, social support services, and optimize use of patient benefits and funding sources (VA, Medicare, self-pay, co-pay and 3rd party insurance) for assigned patients. Technical and Past Performance are significantly more important than price. Solicitations should be available on or about 6/30/01. Closing is expected to be 30 days after that date.
