Description

The Ralph H. Johnson Department of Veterans Affairs Medical Center intends to award a contract for general anesthesiology services. This is considered to be an emergency contract for an approximate 6 month time frame with an additional 6 month option for two board certified anesthesiologists. This is a combined synopsis/solicitation for a commercial item service prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; and a written solicitation will not be issued. The solicitation number is RFP 534-69-01 and is as a request for proposal. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-26. This procurement is Unrestricted NAICS 621111. Contactor shall provide anesthesiology services for this Medical Center on an hourly basis from date of award/credentialing/privileging for a six month period with one six month period option. This is to be a firm fixed price contract with an all inclusive hourly rate. Services will be five (5) days per week, 7:00 a.m. until 3:30 p.m., Monday through Friday, with on-call approximately 7 days per month and one weekend (Saturday and Sunday) per month. Please address an overtime rate for anytime worked over 8 hours per day. Curriculum vitae should be included with credentials. Payments will be on certified invoices in arrears. Offeror must include a completed copy of the provision 52.212-3 Offeror Representation and Certifications -- Commercial Items with its offer. Original proposals shall be submitted on company letterhead or on Contracting Officer's preferred form (SF1449) to the address above before June 20, 2001 at 9:00 a.m. (local time).
