- This is a combined synopsis/solicitation for commercial items = prepared in accordance with the format in FAR subpart 12.6, as = supplemented with additional information included in this notice. This = announcement constitutes the only solicitation; proposals are being = requested and a written solicitation will not be issued. This = solicitation is issued as Request for Quotation (RFQ) No. = F49642-01-T-0192. The solicitation document and incorporated = provisions and clauses are those in effect through Federal Acquisition = Circular (FAC) 97-26. This is a small business set-aside. The North = American Industry Classification System code is 221330 (formerly SIC = code 4961). LINE ITEMS: 0001 Repair Three Geo-Thermal Units, QTY: 1/JB, = UNIT PRICE: $_________, EXTENDED PRICE: $_________. 0002 Installation = of Pete Ports, QTY: 1/JB, UNIT PRICE: $_________, EXTENDED PRICE: = $_________. 0003 Installation of Access Doors, QTY: 1/JB, UNIT PRICE: = $_________, EXTENDED PRICE: $_________. DESCRIPTION/SOW: GENERAL = INFORMATION. The contractor shall provide all management, tools, = supplies, equipment, transportation and labor necessary to repair the = Geo-thermal units in three (3) quarters; install PETE PORTS in twenty = eight (28) quarters; and install access doors in thirty one (31) units = at Hickam Village, Military Family Housing located at Bolling AFB, = Washington D.C. 1.1. The contractor shall repair the leak in the = loop system and correct the pump wiring interlock located at 163 = Clagett Street. The unit Geo-Thermal system will be brought up to = completely operation and industry standards. The contractor shall make = any and all repairs to Geo-Thermal units at 222 Burwell and 339 = Vandenberg to complete installation and make Geo-Thermal units = completely operational. The contractor shall install two (2) Pete Ports = in the following units: 277, 386, 387, 388, 377, 378, 379, 360, 361, = 362, 354, 355, 356, 357, 253, 224, 254, 265, 258, 259, 260, 262, 264, = 265, 266, 178, 179, 190. The contractor shall install access doors on = the return plenum duct in the above-mentioned quarters to allow = inspection of the refrigeration coil entering the airside of unit. All = work shall be in accordance with the statement of work, the appropriate = equipment manufacturers recommendations and commercial practices. 1.2. = The contractor shall coordinate the initial scheduling of work with the = housing office liaison personnel, Quality Assurance (QA) personnel, and = the Contract Administrator prior to commencing work. After the = initial notification to all housing residents, it shall be the = contractor responsibility to notify and follow that schedule with the = housing occupants there after. The contractor shall be responsible for = insuring that they follow and notify all occupants three (3) working = days in advance of the actual day of work. The first reschedules after = the initial attempt and one reschedule that unit/s shall go to the end = of the list. Once all housing units have been contacted and all Pete = ports and access doors are or are not installed there shall be one = final reschedule date. 1.3. The contractor shall maintain = records/logs on housing units completed and those that require = rescheduling. The contractor shall provide a copy of their daily work = log to the QA. As a minimum the log shall have the date, time of = arrival and departure and name of the mechanic/s. FAR PROVISIONS: The = provisions at 52.212-1, Instructions to Offerors-Commercial Items and = 52.212-3, Offerors Representation and Certifications-Commercial Items = are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to = this acquisition is as follow: 52.252-1, 52.252-5. Add paragraph (t): = The Government will award a contract resulting from this solicitation = to the responsible offeror whose offer conforms to the solicitation and = is determined most advantageous to the Government. Contractors = proposal must include companys name, address, and telephone number. = All responses must be received by close of business on 18 Jun 01. Award = will be made on an all or none basis. Offeror must include, with its = offer, a completed copy of the provision at FAR 52.212-3. FAR CLAUSES: = The clause at 52.212-4, Contract Terms and Conditions-Commercial Items = is hereby incorporated. Addendum to FAR Clause 52.212-4 apply to this = acquisition, specifically the following cited clauses: 52.219-6, = 52.252-2, 52.252-6, 252.204-7004, 252.212-7001 (DEV), 252.225-7001, = 252.243-7001, and 252.246-7000.
