Description
The contractor shall provide a certified Optometrist to examine VA patients for the purpose of refracting their eyes and writing eyeglass prescriptions. Examinations will be conducted at the optometrist's place of business, which must be within a ten-mile radius from the perspective VA Community Based Outpatient Clinic (CBOC). The three CBOCs are located in Brick, Hackensack, and Trenton, New Jersey. The qualifications of personnel performing the examinations are subject to review by the VA New Jersey Healthcare System (VA). Requests for services will be made by referral from the primary care physician at the CBOC. It is preferably if contractor can service all three CBOCs but not required. Contractor will be required to perform a complete exam on each patient that includes the following: a) history, b) refraction, c) assessment of extra-ocular muscle movements and pupils, d) visual field screening, e) slit lamp exam, f) tonometry, g) dilated fundus exam, h) frame measurements (for selected frames available on VA contract), i)completion of VA eyeglass prescription form or vendor form containing the same information, j) if applicable, a pathology report should be reported to the referring physician and the Eye Clinic at the East Orange Campus of VA New Jersey Healthcare System. The contractor will provide a sufficient block of time on a daily basis for scheduling veteran's appointments. The estimated number of veteran patients to be seen on a monthly basis follows: a) Brick: 100 b) Hackensack: 100 c) Trenton: 50 This contract is effective from the award date to September 30, 2001 plus 4 one-year options that may be exercised by the VA. The contractor shall not perform services after September 30 each year until the contracting officer authorizes such services in writing.
