Description

Under the authority of Public Law 104-262 and 38 USC 8153 the Department of Veterans Affairs Medical Center, San Juan, Puerto Rico has the requirement to purchase Homemaker services to veterans referred by the Department of Veterans Affairs in accordance with the terms and conditions specified on the solicitation RFP672-293-01. The Contractor shall be required to provide services to all patients referred for services who reside in Puerto Rico. The Contractor shall provide all services identified herein at the patient's residence. All costs associated with transportation of the Contractor or subcontractor personnel shall be the responsibility of the Contractor. Contract award will be based upon the evaluation of offers using numerical scoring, based upon technical excellence, past performance and Price. The period of service will be for a basic period plus two pre-priced one-year options for renewal. This solicitation is set-aside for Small Business. The NACIS number is 624120. The request for proposal (RFP) solicitation package number is RFP672-293-01, and must be requested in writing to: VA Medical Center, Contracting Office (BO-3), 10 Casia Street, San Juan, P.R. 00921-3201.
