Description
DESC: The FISC Norfolk Acquisition Group is issuing a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are hereby requested. A written solicitation will not be issued. This is issued as a Request for Quote (RFQ), Number N00189-01-T-0663 which incorporates provisions and clauses in effect through Federal Acquisition Circular 97-23. This requirement will be 100% Small Business Set Aside. The Standard Industrial Classification Code (SIC) will be 8322, size standard $5 million. This requirement is for one (1) year from 01 October 2001 through 30 September 2002 with four (4) option years. Please provide prices for the following CLINs: CLIN 0001, 1 Lot, Service for Civilian Employment Assistance Program (CEAP), per Statement of Work, Base Year/ 01 October 2001 through 30 September 2002; CLIN 0002, 1 Lot, Service for Civilian Assistance Program (CEAP), Option Year I, 01 October 2002 through 30 September 2003; CLIN 0003, Service for Civilian Employment Assistance Program (CEAP),Option Year II, 01 October 2003 through 30 September 2004; CLIN 0004, 1 Lot, Service for Civilian Employment Assistance Program(CEAP), Option Year III,01 October 2004 through 30 September 2005; CLIN 0005 1 Lot, Service for Civilian Assistance Program(CEAP), Option Year IV, 01 October 2005 through 30 September 2006; STATEMENT OF WORK CIVILIAN EMPLOYEE ASSISTANCE PROGRAM 1.0 INTRODUCTION. The Civilian Employee Assistance Program (CEAP) is a federal government program designed to provide identification, preventive counseling and referral to local community agencies for treatment, rehabilitation, or other assistance for Federal employees/family members with alcohol, drug, or other personal problems. The CEAP will be provided to the Southern Division, Naval Facilities Engineering Command (SOUTHNAVFACENGCOM). The program will include all subdivisions/serviced activities (Officers in Charge of Construction/Resident Officers in Charge of Construction/Public Works Offices, Caretaker Site Offices, Engineering Field Activity Midwest, and Engineering Field Activity Southeast. Total number of civilian employees is approximately 875. Family member is interpreted to mean immediate dependents of employees, i.e. spouse, dependent children, or other immediate family member who qualifies as a dependent for Federal/State income tax purposes. Service is to be provided to the number of employees (plus or minus 10%) listed in the following locations: SOUTHNAVFACENGCOM, Charleston, SC (528); ROICC Albany, GA (8); ROICC Atlanta, Marietta, GA (4); ROICC Barksdale AFB, LA (5); ROICC/OICC Beaufort, Port Royal, SC (20); ROICC Charleston, SC (28); OICC/ROICC JRB, Ft. Worth, TX (14); ROICC Gulfport, MS (23); ROICC Mayport, FL (9); ROICC Orlando, FL (5); OICC/ROICC/PWO Kings Bay, GA (14); ROICC Key West, FL (8); ROICC Memphis, Millington, TN (10); ROICC Meridian, MS (6); ROICC New Orleans, LA (13); ROICC Panama City, FL (5); ROICC Pensacola, FL (21); ROICC South Texas-Corpus Christi (10); Ingleside (6); Kingsville (5); Engineering Field Activity Midwest, Great Lakes, IL (52); Engineering Field Activity Southeast, Jacksonville, FL (58); ROICC Crane, IN (18); Caretaker Site Offices Charleston, SC (3); Louisville, KY (2). 2.0 SCOPE. The specific services to be provided include intake counseling, referral, follow-up, and crisis counseling on virtually any kind of personal problem which affects on the job performance or conduct. Services will be provided either in person or by toll free telephone at the employee/dependent's option for all participants. Employees and their dependents are covered under this program during duty and off-duty hours, as long as the employee is employed by SOUTHNAVFACENGCOM or one of its subdivisions/serviced activities. 2.1 LABOR CATEGORIES. The Contractor shall have licensed, professional diagnostic and referral counselors available in Charleston SC and all outlying areas where subdivisions/serviced activities are located. At a minimum, the Contractor must have all of the following available on its staff to perform intake/triage functions: a. Licensed clinical psychologist with background in substance abuse. b. Licensed clinical social worker with background in substance abuse. c. Licensed marriage, family, child counselor. d. Licensed physician in psychiatry, board eligible. 2..2 LICENSE REQUIREMENT. In addition, the Contractor must be licensed properly as required by the State of South Carolina and all other states included in the SOUTHNAVFACENGCOM areas of responsibility described in paragraph 1.0. 3.0 APPLICABLE DOCUMENTS. Alcohol and Drug Abuse Programs, Office of Civilian Personnel Management Instruction 12792.1, Department of the Navy Civilian Employee Assistance Program. 4.0 TECHNICAL REQUIREMENTS 4.1 INTAKE COUNSELING. Upon receiving a request for assistance, the Contractor shall conduct intake counseling as follows: a. Determine that the problem presented by the employee or dependent falls within the scope of the program; b. A qualified counselor shall interview the participant; c. Screening and diagnosis of participant's problem(s) and assistance requirements; d. Evaluation of the participant and consultation, if necessary, with a specialist in the problem area (if necessary, medical evaluation and testing to be included); e. Obtaining employee authorization to release information in the case of supervisory referral; f. Providing feedback on the employee/participant to the referring supervisor; g. Creation/maintenance of confidential patient records; and h. Explaining the need for and recommending, if necessary, specialized assistance. Intake counseling shall consist of as many sessions as are determined necessary by the Contractor to accomplish the above without cost to the employee. It is anticipated that only in rare cases will more than three counseling sessions per problem be required. 4.2 REFERRAL. When intake counseling has resulted in a determination that treatment or rehabilitation services are required, the Contractor shall refer the participant to appropriate resources which will provide services on a fee basis. The fees will be established between the participant and the service and will be paid by the employee. If a no-fee or low-fee service is available with no significant compromise in quality, referral to this service will be attempted first. This includes service available through/covered under the employee's health benefits plan. The Contractor will maintain current information on health benefits available for federal employees. 4.3 FOLLOW-UP. If referral is made, the Contractor shall follow up with the employee to ensure quality of treatment by the agency referred to and to ascertain status of problem resolution. The Contractor shall also follow up to ascertain if the client participated in treatment as recommended. In the case of supervisory referrals and given the employee has agreed, the Contractor shall provide the supervisor with information on the employee's attendance and the prognosis. 4.4 TRAINING 4.4.1 GUIDANCE FOR SUPERVISORS. When requested, the Contractor shall provide one-on-one guidance and advice to supervisors and managers on the most effective ways to approach an employee with a problem affecting work performance or conduct in order to make effective referrals for assistance. 4.4.2 USE OF PROGRAM. The Contractor shall provide formal training for supervisors and employees to SOUTHNAVFACENGCOM and all SOUTHNAVFACENGCOM subdivision/serviced areas concerning the proper use of the program. The Contractor shall provide a separate training session for all supervisors covered by this contract to familiarize them with the program and the proper procedures. All sessions will be held on mutually agreed upon dates within the first 60 days of the beginning of the contract and whenever determined necessary by the CEAP Coordinator. The Contractor will familiarize employees with the program through distribution of off-the-shelf brochures and posters. The Contractor shall also provide information regarding services provided to supervisors and employees in response to telephone or personal inquiries. 4.5 PRINTED MATERIAL. Written information in the form of existing off-the-shelf brochures shall be provided and distributed by the contractor to each employee at the locations provided in paragraph 4.5a below. Posters for official bulletin boards shall initially be provided to SOUTHNAVFACENGCOM, subdivision/serviced areas within 30 days of the contract award date. Thereafter, printed materials shall be provided as determined by the CEAP Coordinator, but not to exceed 750 brochures or 20 posters in any calendar year. The written contents of the brochures shall contain, at a minimum, specific information about the availability and value of CEAP counseling, encouragement for participation, the objectives of the counseling, and the name, address and telephone number of the Contractor. 5.0 REPORT; DATA DELIVERABLES 5.1 STATISTICAL REPORTS. Quarterly and annual statistical reports of program activity will be provided in written form. The data required will be provided to contractor, via his agent, by the activity CEAP Coordinator and shall contain data necessary to complete Federal Employee Assistance Program Report (Worksheet, Civilian Employee Assistance Programs, Semi-annual and Annual Report), or any successor reports. The Contractor shall monitor the total staff years expended in Program Management and Counseling Services; i.e., actual number of full-time and part-time Administrators and Counselors working with the contract during the year. The format of the current report is provided in Appendix A. The CEAP coordinator reserves the right to request the information in a different format or request additional information. (See OCPMINST 12792.1 for additional formatted reports.) 5.2 ADDITIONAL DATA. The Contractor shall furnish additional data required for any annual or one time report, even if such data is requested after the expiration of the contract. Data shall be provided in writing within five calendar days after request by SOUTHNAVFACENGCOM, EFA Midwest, or EFA Southeast. Otherwise, SOUTHNAVFACENGCOM, EFA Midwest, or EFA Southeast may receive data telephonically within 48 hours after a telephone request. 6.0 SPECIAL CONSIDERATIONS 6.1 CONFIDENTIALITY. Access to the Contractor for counseling is strictly a confidential matter; prior to knowledge or approval of a supervisor, CEAP Coordinator, Contracting Officers Representative (COR), the Human Resources Office, or anyone else, is not necessary. 6.2 REQUEST FOR ASSISTANCE. Service shall be made available by toll free telephone call seven days a week, twenty-four hours a day. A personal assessment meeting will be scheduled at a time convenient to the employee requesting the assistance. Requests for assistance shall be accepted directly from civilian employees and their immediate dependents on their own behalf. Supervisors may obtain advice and guidance from program practitioners. If an employee is referred to CEAP by a supervisor, the employee's participation is strictly voluntary. In the event a counselor is not available to speak with a participant who calls for assistance, a counselor shall return the call within 48 hours. When a participant is involved in a crisis situation, which is defined as any situation perceived by the participant as a crisis, the Contractor shall provide or arrange for immediate assistance. When a group of employees are involved in a crisis situation, the contractor shall immediately provide or arrange group counseling. Callers will be required to identify the organization to which they, or the civilian employee on whom they are dependent, are assigned in order to verify eligibility for assistance. The CEAP Coordinator will periodically provide an alphabetical listing of eligible employees to the Contractor for use in verification of employee eligibility for the use of the program. 6.3 CONFIDENTIALITY OF SPECIFIC RECORDS/DATA. The need for strict confidentiality in a diagnostic/referral counseling program is a prime reason for contracting with a private, independent organization. The Contractor/counselor's records, or any other information pertaining to identity, diagnosis, or treatment of a specific participant, shall be disclosed to SOUTHNAVFACENGCOM or others only on the written and signed consent of the counselee. Such consent shall specify to whom the disclosure shall be made, and shall contain a stipulated period of time for which the release is to remain in effect. As stated above, the Government's access to Contractor/counselor records, except with the written consent of the counselee, is limited to statistical data on the number of cases and the types of problems handled. 7.0 CONTRACTOR FURNISHED MATERIALS. The Contractor will be required to furnish all facilities, supplies, and materials to be used during the performance of this contract except those specified in paragraph 8.0 below. 8.0 GOVERNMENT FURNISHED MATERIALS. Government furnished equipment and facilities will be provided for orientation and refresher sessions and supervisory training sessions.
