Description

THIS IS A SOURCES SOUGHT ONLY- The VA Desert Pacific Healthcare Network (VISN 22), Network Business Center (NBC), Long Beach, CA, is seeking to establish a turn-key capitated rate Outpatient Primary Care Clinic (Community-Based Outpatient Clinic) presence in San Gabriel Valley, CA for the VA Greater Los Angeles Healthcare System (VAGLAHS). The anticipated community-based outpatient clinic shall be located within the city limits of one of the following Southern California San Gabriel Valley cities: (1) Baldwin Park, (2) Covina, or (3) Azusa. The anticipated clinic shall be located along major public transportation routes. The clinic hours shall be established by and based on the VA's workweek Monday through Friday, 8:00 a.m. to 4:30 p.m. There shall be no clinic appointment on federal holidays. The contractor shall be capable of providing primary care services to include blood draw and radiology services. Primary care is generally defined as the delivery and management of primary and preventative medical care and the continuity of that care for all veteran beneficiaries. Population of enrollees is estimated to be 1,000 to 2,000 veterans. The contractor shall provide clinical providers, medical facility for Primary Care, medical equipment and supplies and all administrative functions to fulfill all levels and ranges of care expected for an initial one-year period and four (4) subsequent one-year option renewals. Interested sources are requested to express interest to the Contracting Officer in writing using the company's letterhead and received by 4:00 p.m. PDST on May 31, 2001. E-mail or a fax expression of interest shall also be accepted. The company letterhead must contain a company contact person, telephone and fax numbers, and complete address. The Standard Industrial Classification (SIC) Code is 8011. Small business and small disadvantaged businesses are highly encouraged to participate. VA NBC shall not be responsible for any cost associated with responses to this Sources Sought.
