Description

This Request for Quotation (RFQ) is prepared in accordance with Part 13, Simplified Acquisition Procedures of the Federal Acquisition Regulation (FAR). The contract resulting from this RFQ will be a firm-fixed priced non-personal health care services contract under which the contractor is an independent contractor. The Internal Revenue Service is seeking the services of a local health care facility to provide all personnel, labor, materials, and equipment necessary to accomplish the following medical services: basic medical history and physical examinations, pelvic examinations (including PAP Smear), electrocardiogram, baseline mammogram, and rectal examination and test for occult blood in stool samples for qualified Internal Revenue Service Fresno Service Center employees. The contractor_s facility shall be operated by a State of California licensed medical physician. All medical examination services shall be performed at the Contractor_s facility. The areas of consideration are the Fresno, CA and Clovis, CA areas only. Offeror_s shall be required to provide documentation showing evidence that their facility meets all qualifications to perform the required services within the State of California prior to contract award. The contract resulting from this RFQ will consist of one three-month base period with four twelve-month option periods if exercised. The period of performance for the base period will run from 01 July 2001 through 30 September 2001.
