Description
This is a combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Quotation (RFQ) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-24 and VAAR 97-8. Solicitation number RFQ 652-263-01 should be referenced on all proposals. The NAICS Code for this procurement is 621111 and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. This is a requirements contract. The VA Medical Center intends to solicit quotes and award a firm fixed price contract under the authority of Title 38 USC Section 8153, pending availability of funds. The Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd., Richmond, Virginia has an EMERGENT need for two(2) Board Certified Locum Tenens Radiologist. The Radiologist shall provide coverage 40 hours per week Monday through Friday, 8:00 a.m. -- 4:30 p.m., excluding Federal Holidays from June 4, 2001 through September 30, 2001. No overtime or on call duties are required. Price quoted shall be all-inclusive of all costs, such as, Transportation, Housing, credentialing/privileging and malpractice. No payment will be made for the scheduled half-hour lunch break. Scope of practice will involve the following services: Normal Tour of Duty shall include Plain Film, CT, US, MRI, routine Barium Fluoroscopy, Angio/Interventional procedures and interpretation, as well as, appropriate interaction with Department Staff and Clinical Staff. As a general, Radiologist will be expected, at a minimum, to be able to complete the following studies during any given 8 am to 4:30pm workday, or to stay later than 4:30pm on personal time(normally you will be assigned to only one of these work areas per day): a. Plain films- 75-80 cases (please self-assess your productivity at lunchtime and the end of the day); b. GI/GU-12 cases with a resident (plus 20 plain films or 4-5 body MRI cases); c. CT-15 patients, plus 1 CT guided procedure, with a resident; d. US-19 patients,( Plus 20-25 plain films or 4-5 Body MRI cases); e. Angiography-5-6 patients; f. CT/US-12 US cases and 8-9 CT cases, with a resident; g. Neuroradiology-13-17 CT and/or MRI cases and 2 myelograms or 12-15 CT and/or MRI cases and 1-2 carotid angiograms. He/she will only be assigned those areas and duties that they have specified on their application. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, and income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. The Special Contract Requirements for Enhanced Healthcare Resources Sharing Agreements is incorporated in this solicitation; a copy is available upon request. Offerors responding to this announcement shall submit a cost proposal and technical proposal to include the following evaluation factors: Candidates experience and personnel qualifications to include evidence of current licensure and credentials/privileges and current CV of candidate, Indemnification and Medical Liability Insurance certificate, past performance of contracts of similar type, scope size and complexity that are ongoing or completed within past three years, references with points of contacts for contracts identified. Offers that do not include CV's of Candidate presented WILL NOT be considered. Offerors may propose more than one candidate. CANDIDATE MUST BE AVAILABLE FOR IMMEDIATE DUTY-PERIOD OF PERFORMANCE TO BEGIN NOT LATER THAN June 4, 2001. The Cost Proposal shall include: (1) Hourly Rate for Normal Business hours $__ ____. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. All documentation required to credential and privilege the physician must be provided to the Credentialing Office at this Medical Center prior to contract award. OIG List of Excluded Individuals/Entities applies. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting officer. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items, 852.219-70, Veteran Owned Small Business certification must be submitted with offer.
