Description
A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.5 as supplemented with additional information included by electronic notice. Electronic solicitations may be obtained at the following URL: http/www.va.gov/oa&mm/busopp/sols.htm. Any amendments issued to this solicitation will also be available at the URL address. The electronic announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. Offerors are advised that they are responsible for obtaining amendments. The Department of Veterans Affairs, White River Jct., VT is seeking the services of 2 ICU nurses. It is the intent of this contract to obtain ICU nurse coverage until permanent placements can be obtained. Work is to be performed at the VA Medical and Regional Office Center, White River Jct., Vt, for a four month period with options for extending this contract beyond that date. These individuals shall work in a 7-bed medical/surgical ICU with an average patient census of 5-7 patients per day. The VA serves both male and female veterans, but the clientele is predominately male. Patients typically cared for in the ICU are suffering from multi-system failure. Typical diagnoses of medical patients seen in the ICU are Acute Myocardial Infarction, COPD and respiratory failure requiring mechanical ventilation, acute strokes, and diabetic ketoacidosis. The nurses shall work rotating 8-hour shifts, 40 hour weeks. The current rotation is 3-4 shifts in a row, have a day off, work 3-4 more shifts have a day off and then work another 2-3 shifts and have 2 days off, inclusive of weekends and holidays. The solicitation number is 405-37-01. This solicitation is issued as an RFP. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-24. The acquisition is unrestricted. The NAIC code is 621999. Evaluations will be made on availability, qualifications, past performance, price, and small disadvantaged business HUB Zone participation. Offerors are required to submit a complete copy of the provision at 52.212-3 -- Offerors Representation and Certifications -- Commercial Item with their offer. No telephone requests will be accepted.
