- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and Part 13, Simplified Acquisition Procedures, as supplemented with additional information contained in this notice. This notice incorporates provisions and clauses in effect through FAC 97-25. NAIC Code 56320 applies and the small business size standard is 10 million. This announcement constitutes the only solicitation. A Written solicitation WILL NOT be issued. This solicitation is issued as a Request for Quotations (RFQ). The St Cloud VA Medical Center, 4801 8th Street North, St Cloud, MN 56303-2099, has determined an urgent requirement for Temporary Diagnostic Sonographer; on Mondays and Thursdays, 8:00 AM to 4:30 PM schedule. The VA will require a minimum seventeen weeks of Monday, Thursday work starting June 4th through September 27, with possible extended coverage. Work is to be performed in the Radiology Department, Bldg. 1, of the above named facility.Individual must be RDMS eligible or registered in Diagnostic Medical Sonography in at least one specialty area. The following is a list of duties and responsibilities:(1) Performs vascular ultrasound examinations and must have complete and thorough knowledge of cross-sectional anatomy of the carotid arterial, peripheral arterial and venous system.(2) Performs ultrasonic imaging studies for visualization and determination of organ position, form structure, pathology, localization processes and midline studies. These studies include: abdominal: liver, gallbladder, spleen, pancreas, abdominal and thoracic aorta, vena cava and femoral arteries, etc.Retroperitoneal: kidneys, adrenal glands, nodes, and urinary bladder.Small parts: thyroid glands, parathyroids, salivary glands, scrotum, etc.Pelvis: prostate (transabdominal), transvaginal, bladder, lymph nodes, etc. (3) Adjusts equipment controls to proper setting, select, apply and position probes on patient's body depending on incumbents instantaneous recognition of produced images to obtain the proper penetration producing the best sonogram (sonographic tracing) to make proper diagnosis.(4) Prepares patients for examination by explaining and teaching the procedures to the individual undergoing the study as well as seeking out pertinent clinical history or additional clinical history for correlation of the test to be performed by thorough chart review, including any necessary consultation with the referring physician regarding a patient's history and the decision of appropriate ultrasonic procedures to be performed in the role of advising the referring physician as the availability of different types of studies and the most proper utilization of such studies.(5) Interprets the test performed as to provide a determination concerning the acceptability or rejectability of sonographic scans in terms of the diagnostic usefulness for the radiologist's ultimate diagnostic conclusion.6) Assists diagnostic radiologic technologists in areas where trained to-do-so as time permits. Physical demands; the work requires long periods of standing and walking. There is some bending and carrying of moderately heavy articles such as equipment or records. Patients sometimes are assisted to achieve proper positioning which may require reaching or work in strained positions. Contractor's employees must be present at the VA and must be performing the required services or the contract cost will be decreased accordingly. The 30-minute lunch period will not be compensated. Monitoring of contractor's time shall be demonstrated from work. Payments to contract personnel for any leave, including sick leave, holiday, or vacation time are the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor's policies. The contractor shall be responsible for protecting the personnel furnishing services under this contract. The contractor shall provide workers compensation, income tax withholding, social security payments, etc. The government may evaluate the quality of professional and administrative services provided, but retains no control over the professional aspects of services rendered. As part of the pre-award contractor responsibility determination and during the performance of the contract for the purpose of on-going contractor competency evaluation, the VA will perform or require for evaluation from offerors the submittal of one or more of the following: (a) information to allow evaluation of qualifications and past performance. Information should be specific, concise, complete, and sufficient enough to demonstrate offeror's capacity to meet or exceed the requirements of the contract; (b) evidence of accomplishment of training and education; (c) evidence of experience and achievements in work similar to that required by the contract; and (d) certification or licensure that meets or exceeds standards relative to the industry being contracted. Contractor shall provide the following information along with offer for each candidate: (A) Identification information (work address, DOB, SSN#, home address, etc.); (b) licensing information (State, number, expiration date); (c) Education (including Professional training); and (d) references and phone numbers. Substitutions shall have appropriate qualifications and VA reserves the right to request replacement, if necessary. Award will be made to the lowest priced, responsible, responsive offeror. Award may be made with or without discussions. All responsible sources above may respond to this solicitation by submitting cost and bid form.
