Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1.The request for quotes (RFQ) solicitation number is RMAST-01-153. 2.The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-25. 3. This is NOT a small business set aside. The associated NAICS code is 621210 and small business size standard is $5.0 million 4. Dental Services on the Trapper Creek Job Corps Center outside Darby, MT in accordance with the attached specifications. SCHEDULE OF ITEMS Quantities shown are estimated and not guaranteed. Payment will be based on actual quantities Base Year -- July 1, 2001 through June 30, 2002: Estimated 48 days service at $_______________per day. Option Year One -- July 1, 2002 through June 30, 2003: Estimated 48 days service at $________________ per day. Option Year Two -- July 1, 2003 through June 30, 2004: Estimated 48 days service at $________________per day. BASIS OF AWARD: A PURCHASE ORDER WILL BE ISSUED TO THE QUOTER WHOSE QUOTE IS MOST ADVANTAGEOUS TO THE GOVERNMENT, PRICE AND THE ATTACHED FACTORS CONSIDERED. A PURCHASE ORDER MAY BE ISSUED BASED ON INITIAL INFORMATION SUBMITTED, WITHOUT ANY FURTHER DISCUSSIONS. COMPLIANCE WITH VETERANS EMPLOYMENT REPORTING REQUIREMENTS (52.222.70)(JAN 1999)(DEVIATION) (a) The Offeror represents that, if it is subject to the reporting requirements of 38 U.S.C. 4212(d)(i.e., the VETS-100 report required by FAR clause 52.222-37, Employment Reports on Disabled Veterans of the Vietnam Era), it has [ ], has not [ ], submitted the most recent report required by 38 U.S.C. 4212(d). (b) An Offeror who checks "has not" may not be awarded a contract until the required report is filed. SPECIFICATIONS -- There are specifications for these services. EVALUATION -- COMMERCIAL ITEMS (52.212-2)(JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: [a] Professional Qualifications, [b] Specialized experience and technical competency, [c] Capacity to perform the requirements of the contract within the time frames and locations specified, [d] past performance on similar contracts, [e] Ability and willingness to work with disadvantaged adolescents and young adults in a positive and respectful manner, and [f] Proximity of the contractor to the Trapper Creek Job Corps Center. Offerors should provide a narrative addressing each evaluation factor. Technical and past performance, when combined, are equal in importance when compared to price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders -- Commercial Items (Feb 2001), applies to this acquisition.
