- Description
- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is DAKF61-01-Q-0052 and is a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-25. (iv) This acquisition is set-aside for small business concerns and the associated NAICS code is 212319 and small business size standard is 500 employees. Payment will be made via the Government VISA card and the contractor is responsible for all associated processing costs. There are six contract line items (CLINs). All deliveries will be made to Fort McCoy, WI to sites specified and will have contractor weigh slips delivered with each shipment. Prior to delivery contact will be made with specified POC at Fort McCoy. American Association of State Highway Transportation Officials (AASHTO) standard test results required for CLINs specified. The contractor shall furnish certified reports of tested aggregates prior to start of contract. Exact map locations available upon request by contacting the Contract Specialist via e-mail. CLINs are as follows: (0001) Crushed Rock for aggregate base course WIDOT Gradation #3 (AASHTO Test Results Required), delivered and stockpiled at the North Post Concrete Dump -- Delivery Date Between 11 June -- 15 July 2001 (Quantity = 5700 Net Ton); (0002) Crushed Rock for aggregate base course WIDOT Gradation #3 (AASHTO Test Results Required), delivered and stockpiled at the South Post Quarry Hill -- Delivery Date 11 June-15 July 2001 (Quantity = 1950 Net Ton); (0003) Crushed Rock, WIDOT #1 Gradation (AASHTO Test Results Required), delivered and stockpiled at North Post Concrete Dump -- Delivery Date Between 11-15 June 2001 (Quantity = 1800 Net Ton); (0004) Riprap, 6" Limestone WIDOT standard, delivered and stockpiled at the North Post Concrete Dump -- Delivery Date 11-15 June 2001 (Quantity = 1600 Net Ton); (0005) Riprap, Pit Run Shot Rock from State Certified Pit-certified pit results required, Fines to 3" -- 4" chunks (Need larger flat pieces) -- delivered within five miles of the Post Stockpile 700 Block -- Delivery between 11-15 June 2001 (Quantity = 700 Net Ton); (0006) Riprap, 8" to 12" WIDOT standard, delivered and stockpiled at the North Post Concrete Dump -- Delivery between 11-22 June 2001 (Quantity = 1600 Net Ton). (viii) The contractor agrees to comply with all provisions and clauses incorporated in this acquisition. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial (no addenda). (ix) The provision at FAR 52.212-2 Evaluation-Commercial Items is included. Paragraph (a) is completed as follows: the factors to be used in the evaluation of offers are (1) technical capability of the contractor to meet stated requirements of the end item, (2) past performance and (3) price. Paragraph (b) is completed as follows: technical capability and past performance when combined are approximately equal to price. To evaluate technical capability the offeror shall submit (AASHTO) test results from gravel pit to be used and a list of equipment that will be available for use by the contractor in performing the contract. References including the business name, a point of contact with phone number and a brief description of similar work performed within the past three years shall be submitted to evaluate past performance. (x) FAR 52.212-3, Offeror Representations and Certifications-Commercial Items with Alt I & III are required for this acquisition. (xi) FAR 52.212-4, Contract Terms and Conditions-Commercial Items applies (no addenda). (xii) Also applicable to this acquisition are the following: FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items with the following applicable clauses in paragraph (b): 52.203-6 Restrictions on Subcontractor Sales to the Government with Alternate I; 52.219-8, Utilization of Small Business Concerns; 52.219-14 Limitations on Subcontracting; 52.222-21 Prohibition of Segregated Facilities; 52.222-26 Equal Opportunity; 52.222-35 Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36 Affirmative Action for Workers with Disabilities; 52.222-37 Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.222-19, Child Labor-Cooperation with Authorities and Remedies; and 52.232-36, Payment by Third Party. The clause at 52.219-6 Notice of Total Small Business Set-Aside applies. Also applicable are the following DFARS Provision and Clauses; DFARS clause 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, with the following applicable clauses in paragraph (a): 252.225-7001 Buy American Act and Balance of Payments Program; 252.225-7012, Preference for Certain Domestic Commodities; 252.243-7002, Requests for Equitable Adjustment; and 252.247-7023, Transportation of Supplies by Sea. DFARs clause 252.225-7000, Buy American Act-Balance of Payments Program Certificate; 252.225-7002, Qualifying Country Sources as Subcontractors; and 252.212-7000, Offeror Representations and Certifications Commercial Items are also applicable. Alternate Disputes Resolution will be used to resolve issues in controversy. In accordance with FAR 39.106, the contractor shall ensure the offered product and all information technology (IT) contained therein shall be Year 2000 Compliant. PARTIES INTERESTED IN RESPONDING TO THIS RFQ may submit their quote in accordance with standard commercial practice (i.e. quote form, letterhead, etc) and MUST INCLUDE THE FOLLOWING INFORMATION: Solicitation Number (DAKF61-01-Q-0052), offeror's complete mailing and remittance addresses, discount terms, unit price per CLIN, terms of any express warranty, DUNS number, and COMPLETED FAR 52.212-3 and DFARs 252.212-7000.
