Description

The Great Lakes Acquisition Center (GLAC) is soliciting quotations to provide VA Hospital Tomah, WI., Outpatient Primary Care Services to its beneficiaries in the Wausau, Wisconsin area. The patient population is currently approximately 1,250 beneficiaries. Location of services will be within a ten- mile radius of the city of Wausau, Wisconsin. Period of performance will be a base year plus (4) one year options, at the Government's discretion. Contract type will be firm-fixed price. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof. The NAICS number is 62111. The successful offeror shall furnish health care providers, medical facilities, medical equipment and supplies, and all administrative functions to fulfill the level of care and range of services necessary to meet the primary medical health needs of the enrolled Department of Veterans Affairs beneficiaries. This will include initial prescriptions and drugs/medications. It is anticipated that the date in which services will begin is September 04, 2001.
