Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUES THE ONLY SOLICIATION: QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number Request For Proposal (RFQ) 623-18-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification Code is 621999 and the small business size standard is 5 million dollars. The Muskogee VAMC, located at 1011 Honor Heights Drive, Muskogee, Oklahoma 74401, has a requirement for three Radiology diagnostic technologists and one ultrasound technologist. Contract employees are to work Monday through Friday 8:00 A.M. 4:30 P.M. excluding Federal Holidays. Persons performing work on this contract shall be available to take call during, holidays, off hours, and weekends. Contract duration is from date of award through September 30th, 2001.DIAGNOSTIC TECHNICIANS Shall be able to: 1. Perform routine radiographic examinations of body parts such as chest, skull, abdomen and the appendicular organs. 2. Perform fluoroscopic examinations under supervision of radiologist, examinations such as esophagus, Upper Gastrointestinal, barium enema and T-tube cholangiograms. 3. Perfrom various portable examinations and operating room studies. I.e. C-arm fluoroscopy, etc.. 4. Perform studies on an average of 20-25 patients per day. ULTRASOUND TECHNICIAN -- 1. Have thorough knowledge of principles, techniques and methods of ultrasonography and non-invasive examinations with the use of high frequency sonography. Performs abdominal ultrasound examinations of specific organs; gallbladder, liver, aorta, kidneys, urinary bladder and transrectal prostate ultrasound studies. Also, endovaginal pelvic examinations, thyroid, testicles, doppler examinations and ankle brachial indexs or other organ systems as required. 2. Performs localization of mass or abnormal area for biopsy or thoracentesis. 3. Performs examination on approximately 15-20 patients per day. Note: Federal holidays observed are Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas, and New Year's Day or any other day declared by the President of the United States to be a national holiday. The contractor shall ensure each contracted staff member provided has the appropriate training and/or education. The Contractor shall keep records and withhold appropriate Federal and State taxes and fees (FICA, SS, income tax, etc.). The contractor shall ensure that he/she receives mandatory training as required by OSHA, JCAHO, etc. The contractor shall provide the VA with check-sheet indicating that the provided persons have undergone the necessary training. The contractor shall ensure proper dress (smock, shirt, pants, shoes, etc.) and conduct of employees. The professional/legal responsibility for services provided resides within the person provided by the contractor. All persons performing work under this contract shall be properly certified/registered/licensed in at least state in the USA. Quotations shall be submitted free of special stipulations. Partial quotations shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, and 52.212-5 also apply.
