This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This requirement is a Brand Name or Equal. If submitting a quote for other than brand specified, technical literature specifications must be submitted with quote. The solicitation number is FU050910920100 and is a Request For Quote (RFQ). The solicitation is 100% set aside for small business. The North American Industry Classification System Code (NAICS) is 333315 and the small business size standard is 500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. Vendor must be actively registered with Central Contractor Registration. Offerors must comply with all instructions contained in FAR 52.212-1, Instruction to Offerors -- Commercial Items, applies to this acquisition and its addenda. FAR 52.212-2, Evaluation -- Commercial Items, also applies. Award of this acquisition will be based on Best Value to the Government and the evaluation criteria for award will be technical proposal, price and past performance. If proposal is technically acceptable, in order of importance, price will be evaluated as significantly more important than past performance. Offerors will provide a list of three (3) of the most relevant (like or similar) services performed for either the Federal Government or commercial customers within the last three years. Offerors must provide current phone, fax or e-mail address of past performance references. References must be submitted along with price quote. Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items. FAR 52.222-41 Services Contract Act of 1965, as amended (May 1989) FAR 52.232.33, Payment by Electronic Funds Transfer, Central Contractor Registration; DFARS 252.204-7004, Required Central Contractor Registration; DFARS 252.212.7001, Contract Terms and Conditions Required to Implement Statutes or executive order Applicable to Defense Acquisitions of Commercial Items; Wage Determination 94-2309, Rev 21, dated 12/22/2000 apply. The requirement is to furnish all labor, equipment, tools, materials, electricity, transportation, and supervision necessary to provide the GTNPMO an H.320 compatible VTC system, commercial ISDN circuit (384K/3 BRI), and associated audio/visual equipment for the new GTNPMO conference room to be constructed in Building 1700. Equipment list and Statement of Work attached. This announcement constitutes the only solicitation; a written solicitation will not be issued. To assure timely and equitable evaluation of offers, each offeror must submit a completed copy of: Representations and Certifications, Technical Proposal; (to include manufacturer's specifications and literature, Price Proposal and Past Performance References.
