Description

This notice is a combined synopsis/solicitation for a commerical item prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. A request for quote must be made in writing to the Contracting Officer and may be faxed to 518-462-0051. This procurement is being conducted under Simplified Acquisition Procedures (SAP). The reference number for this procurement is RFQ #500-33-01 and incorporated the clauses and provisions that are in effect through Federal Acquisition Circular 97-14. Quotes should be submitted for the following item: Argon Plasma Coagulator System, APC 300/ICC200 E-A or equal. The system must include pressure reduction value, trolley including accessory fixation assembly, connecting cable, footswitch connector, reusuable probe connector, electrosurgical generator, double pedal footswitch, adapter for BICAP or goldproble, ignition test, gas tank-full 40 cu ft capacity-99.998% pure, S-cord 10", surge protector, grounding cable set,installation, training, & at least 1 yr warranty. Quotes received shall specify FOB destination. The provisions at FAR 52.212-1 Instruction to Offerors-Commercial Items and FAR 52.212-3 Offerors-Representations and Certifications applies to this acquisition and a completed signed copy of this provision should be submitted with the quote. The Representations and Certifications may be obtained via the Internet at: http://www.arnet.gov. The provisions at FAR 52-212-3 Evaluation-Commercial items also applies to this acquisition. The clauses at FAR 52-212-4 Contract Terms and Conditions-Commercial Items and FAR 52-212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-commercial Items applies to this acquisition. The following factors, in descending order of importance, will be used to evaluate the quotes: (1) determination of product acceptability, (2) lowest priced quotations, and (3) delivery schedule and (4) technical ability.
