- The above-numbered solicitation Request for Proposal (RFP) is hereby amended (Amendment #2) as follows: Item #1 -- The proposal submission date is extended to May 3, 2001, 4:30 p.m. On page 45, Paragraph 2B, Addendum to FAR 52.212-1 Instructions to Offerors -- Commercial Items is deleted in its entirety. Incorporated is the revised page 45. Item #2 -- On Page 29, Deliveries or Performance, Paragraph 2, is deleted in its entirety. Incorporated is the revised page 29 rewriting paragraph 2.Item #3 -- On Page 58 (FAR 52.217-8), Option to Extend Services is deleted in its entirety. Incorporated is the revised page 58. Item #4 -- On Page 59 (FAR 52.217-9)-Option to Extend the Term of the Contract, Paragraph (c) is deleted in its entirety. Incorporated is the revised page 59 changing the "not to exceed years from 15 years to 15 1/2 years. Item #5 -- On Page 4, Description/Specification, is deleted in its entirety. Incorporated is the revised page 4 changing the full time/seasonal employees for IRS Brookhaven Service Center, 1040 Waverly Avenue from 6,120 full time employees to 2,170 full time/ seasonal employees; for 5000 Corporate Drive from 1,725 full time employees to 1,907 full time/seasonal employees. Paragraph 2(a) (3) Performance data for 1040 Waverly Avenue- IRS Brookhaven Service Center (only) is $1,125,786.00 (represents the Sales for Fiscal year ending June 2000). This dollar figure does not include sales dollar amount for 5000 Corporate Drive. The Contractor was not providing service at 5000 Corporate Drive during this period. Item #6 -- On Page 5, paragraph b (3), Performance data for 5000 Corporate Drive, is deleted in its entirety. Incorporated is the revised page 5 to include the sales and Customer Counts for 5000 Corporate Drive as follows: Corporate Drive Sales and Customer Counts: January 2001- Sales $9,800, Customer Counts 6,110; February 2001- Sales $15,500, Customer Counts 10,137; March 2001-Sales $34,000, Customer Counts 21,900. Item #7 -- On page 44, Building Population Projection, is deleted in its entirety. Incorporated is the revised page 44 changing the "Approximately number of full time employees during peak period" from 5,000 to 4,077 full time/seasonal employees. Item #8 -- On pages 61 through 71, Collective Bargaining Agreement (CBA) between Lackmann Food Service and the United Service Employees Union, Local 377, RWDSU, AFL-CO is only applicable for this procurement. All employees are included in the CBA. Pages 72 through 80, Wage Determination No. 1994-2373 are deleted in its entirety. Item #9 -- On page 41, Exhibit C, 1040 Waverly Avenue, Cafeteria Equipment Inventory, is deleted in its entirety. Incorporated is the revised page 41 (with two additional pages, 41A & 41B), Exhibit C, 1040 Waverly Avenue -- Cafeteria Equipment Inventory List which identifies the types of equipment and how many of each. GSA cannot provide a list of equipment that Lackmann owns. Item #10 -- THIS IS IN RESPONSE TO MR. DAWSON'S QUESTION REGARDING DAILY CUSTOMER AVERAGES (SEE PRE-PROPOSAL CONFERENCE MINUTES OF MEETING -- PAGE 24). The Average Daily Customer Counts for Monday through Friday are as follows: Corporate Drive: Breakfast-325; Lunch- 250; PM Break-190; Dinner-185. Brookhaven Service Center: Breakfast -- 850; Lunch-575; PM Break- 425; Dinner-200 Item #11 -- The acquisition costs to the Government for this procurement will not exceed the simplified acquisition threshold. Thus, per FAR 19.1102 and FAR 19.1307 accordingly, the price evaluation adjustment for Small Disadvantaged Business Concerns and the price evaluation preference for HUBZone small business concerns do not apply to this procurement. Item #12 -- Full population (includes 1040 Waverly Ave. facility and 5000 Corporate Drive facility) is 4,077 full time/seasonal employees (See revised page 4). Item #13 -- The full population number from January 1 through May 31 (peak period) is 4,077 full time/seasonal employees, and the full population number for non-peak is 2,800. All that we can provide are the full population number for the peak period employees and the off-peak period employees (See revised page 44). Item #14 -- THIS IS IN RESPONSE TO MR. DAWSON'S QUESTION REGARDING SUBCONTRACTING COST (SEE PRE-PROPOSAL CONFERENCE MINUTES OF MEETING -- PAGES 34 AND 35). The Government does not get involved with the prospective contractor's subcontracts such as trash removal cost. It is the responsibility of the prospective contractors to secure their subcontracts. Item #15 -- On Page 13, paragraph h, Contractor performed repairs, is deleted in its entirety. Incorporated is the revised page 13 revising paragraph h. Item #16 -- On page 11- Paragraph e (2), Personnel and Supervision, is deleted in its entirety. Incorporated is the revised page 11, Paragraph e (2) to include the following sentence: "The manager or his designated person must be onsite at all times that the cafeteria is open for the duration of this contract." Item #17 -- This Solicitation is amended to include all questions and answers recorded at the Pre-proposal conference on March 28, 2001 (See attached sheets). THE ENTIRE REQUEST FOR PROPOSAL (RFP) SENT TO YOU, INCLUDING AMENDMENT #1 AND AMENDMENT #2 COMPRISES THE COMPLETED OFFER PACKAGE, AND MUST BE SIGNED AND RETURNED TO THE GENERAL SERVICES ADMINISTRATION, PROGRAM SUPPORT DIVISION, 26 FEDERAL PLAZA, RM. #18-130 (18TH FL.), NEW YORK, NY 10278 BY CLOSING TIME AND DATE. (May 3, 2001, 4:30 p.m.). ALL OTHER TERMS AND CONDITIONS REMAIN THE SAME.
