Description

The purpose and intent of this contract is to provide the following: Review for clarity and provide advice reguarding quality on video tapes of culverts on Agness Road 33, Gold Beach Ranger District, Siskiyou National Forest. Then provide field visit, design, write specifications, costing, and implementation oversight of pipe ramming at Mile Post 13.28 on the Agness Road. And field visit, design specifications, costing for trenchless treatment of approximately 200 culverts on the Agness Road and provide initial implementation oversight of the trenchless treatments. Work will begin approximately May 1 and will consists of approximately 237 hours spread over 525 calendar days. Firms qualified and interested in providing these services are invited to respond by submitting Standard Form 254 (Architect-Engineer and Related Services Questionnaire) and Standard Form 255 (Architect-Engineer and Related Services Questionnaire for Specific Project) in accordance with the instructions contained in CBD Numbered Note 24. Responses shall be sent to Siskiyou National Forest, 200 NE Greenfield Road, PO Box 440, Grants Pass, OR 97528. Materials will be accepted through April 30, 2001. This procurement is an Emerging Small Business Set-Aside in accordance with P.L. 100-565, Small Business Competitive Demonstration Act.
