Description
MODIFICATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice and following the simplified acquisition procedures in FAR part 13.5. Solicitation number 6TSA-01-JMT-0132 is being issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-14. This procurement is to provide infrastructure upgrades to all USAMH clinics throughout Germany. The identified activities are, but not limited to site surveys, analyzing existing hardware/electronics; providing recommendations for upgrading existing hardware/electronics including all required connections cables; identifying needed configurations/quantity/cables in coordination with U.S. Government and DISA; cabling of network and telecommunication wires; installing hardware/electronics and connecting equipment; documentation and technical assistance. Please note Selection Criteria in the Scope of Work. This procurement is for the U.S. Army Medical Department, Heidelberg, Germany (USAMH). The contract will be awarded based on the proposal for the first site described in the clarification document. The other clinics listed in the Statement of Work will added to the contract as money becomes available to the client. THE CLARIFICATION DOCUMENT DOES NOT REPLACEMENT THE STATEMENT OF WORK. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Evaluation is based on best value including past performance, cost and ability to meet service requirements. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at FAR 52.212-4, Contract terms and Conditions-Commercial Items, applies to this acquisition. In addition to the guidance contained in FAR 52.212-4, contractors shall submit proof of delivery with their invoices. Proof of delivery may be in the form of a bill of lading or other shipping document signed by the receiving office. Invoices received without proof of delivery will not be accepted. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
