THIS NOTICE CONSTITUTES AMENDMENT NO. 4 TO THE COMBINED SYNOPSIS/RFQ FOR EQUAL OPPORTUNITY ADVISORY GROUP CLASS. Companies shall acknowledge all amendment(s) in their quote. This notice serves as the official amendment to subject synopsis/RFQ and a written amendment will not be issued. Companies must only reference this amendment if they are resubmitting a proposal based on a zip code error in Modification 1. The purpose of this amendment is to clarify the mailing address. If a company has mailed a quote that has been returned to them based on the incorrect zip code listed in Modification 1, they may resubmit. Resubmitted quotes will be accepted until 4:30PM 4/16/01. It is mandatory for all resubmitting companies to include documented proof (i.e. a return-to-sender postmark) in order for their corresponding quote to be accepted.
