Description

Contractor shall provide Locum Tenen Pyschiatrist Services for the DVA, Community Based Outpatient Clinic, 3325 Chanate Road, Santa Rosa, CA 95404 for a Base period of three (3) months with four one-month options to renew. This is a commercial procurement. The following clauses and provisions plus any addendum apply: 52.212-1 Instructions to Offerors -- Commercial Items, 52.212-2 Evaluation -- Commercial Items, 52.212-3 Representations and Certifications -- Commercial Items, 52.212-4 Contract Terms and Conditions -- Commercial Items, and 52.212-5 Contract Terms and Conditions to Implement Statute and Executive Orders- Commercial Items.
