- This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal (RFQ) 623-13-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. The North American Industry Classification Code is 62111 and the small business size standard is $5 million. The Muskogee VAMC located in Muskogee, Oklahoma 74401 has a requirement for a Radiologist. The Radiologist is responsible for providing non-interventional radiographic services at the VA Medical Center, Muskogee, Oklahoma, Monday through Friday, 8:00am 4:30pm. The services will include reading and interpreting radiographs in the VA Medical Center, Radiology Service. Services will be required for an estimated 30-35 patients per day of varying modalities. Based on the foregoing estimated patients this equates to an estimated 70-100 radiographs per day that will be read and interpreted. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. The services to be performed by the contractor will be under the direction of the Chief of Staff, and the Chief, Radiology Service. Evaluation/treatment of patients: Evaluates VA patients who are presented for care, which includes the following: a. A detailed and complete reading and interpretation of the radiologic examination will be accomplished consistent with the patient's presentation. b. Effective communication of the reading and interpretation of diagnostic studies of the patient to the referring physician. c. Assures that consultative entries into the radiology record and patient's medical record are in accordance with VHA Directives, Medical Center Memorandums, Medical Staff By-Laws and JCAHO Standards. d. The referring physician will be notified by telephone immediately of patient conditions which require urgent or emergent treatment or intervention by the referring physician and followed by a detailed written or typed report. e. Contractor shall participate in activities of the VA Medical Center that are designed to meet the requirements of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) pertaining to Quality Improvement (QI) for which medical staff are responsible, i.e. maintain radiology records and participate in peer review. The Contractor shall be responsible for assuring that radiology reports are completed in a timely fashion satisfying VHA Directives, Medical Center Policies/Memorandums and JCAHO Standards. The VA Medical Center will provide the following: a. Scheduling of patients for diagnostic radiographs will be performed by the VA Medical Center. b. The VA Medical Center will provide and maintain the patient's Radiology images and report record excluding Contractor's responsibilities previously outlined. c. All radiology examinations performed by the Radiology technologists will be coordinated by the VA Medical Center. d. Support staff (i.e. technologists and administrative) for Radiology Services. Billings will be made in arrears for services rendered. The VA Medical Center, Muskogee, Oklahoma, shall establish and maintain a record keeping system that will record the hours worked by the contractor's employee(s). This contract requires the Contractor's employee, who meets all of the requirements of the contract, to perform the work specified herein. A Contractor's employee must be present at the VA facility and must be actually performing the required services in accordance with contract specifications in order to receive compensation. Any complications, misdiagnosis, and inappropriate care will be recorded. Any incidence of Contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. This contract will be for a period of 1 year, with two option years. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the Chief of Staff and approval by the VA Facility Director. Physicians assigned by the Contractor to perform the services covered by this contract shall be US Citizens and credentialed and privileged in accordance with VA regulations set forth in VA Circular 10-91-070, Credentialing and Privileging of Physicians and Dentists. Physicians must be currently Board Certified in Radiology by the American Board of Radiology or American Osteopathic Board of Radiology and maintain that certification during the duration of the employment. Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein. This contract includes standard hours as well as on-call hours not to exceed a combined total of 2,548 each 12 month period. Call may be scheduled on week-nights after normal hours, weekends, and National Holidays. (1) Standard Work hours: Monday through Friday, 8:00 a.m. -- 4:30 p.m., except National Holidays. (2) National Holidays: The 10 holidays observed by the Federal Government are: New Years Day, Martin Luther Kings Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas AND any other day specifically declared by the President of the United States to be a national holiday. Personnel Policy: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: workers compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. The responsibility for any leave, including any sick leave or vacation time, is the responsibility of the contractor. Contract Performance Monitoring: Monitoring of contractors time shall be demonstrated through sign-in/sign-out sheets. The contractor shall be required to sign an attendance log upon reporting to work and departing from work. Chief, Radiology Service, shall be the VA official responsible for verifying contract compliance. After contract award, any incidents of contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. Substitution of Physicians: Substitution of personnel shall be made only as follows -- a. All proposed substitutions must be submitted in writing at least 60 days in advance of proposed substitution to the Contracting Officer. All proposed substitutes shall have the appropriate qualifications. b. The Contracting Officer shall evaluate requests for substitution of personnel and promptly notify the contractor, in writing, of whether the request is approved or disapproved. Contractor shall submit CV's for all proposed physicians along with a tentative schedule for physician coverage. It is anticipated that award will be made in May 2001 with services starting ASAP thereafter. Quotations shall be submitted on an "all or none basis." Quotations shall be broken down in the following manner: On call per hour price, per day for normal working day, and call-back hour rates annotating the minimum call back hour rate (i.e. minimum call back is two hours @ , etc.). Quotation shall also show lump sum annual cost. The government estimates 2008 hours of standard coverage and 540 hours of on-call coverage. Partial quotations or quotations not priced in this manner shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately.
