Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number RFQ 667-17-01; award will be made using part 13.5 test program for commercial items and the simplified acquisition procedures. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. This acquisition is unrestricted. The North American Industry Classification System # 561320 and the small business is $10,000,000. This procurement is executed under Enhance Sharing Authority, Public Law 104-262, Title 38 U.S.C. 8153. The Contractor shall provide locum-tenens nuclear medicine physician services for the Overton Brooks VA Medical Center, Shreveport, LA in accordance with all terms, conditions, provisions and the schedule of this solicitation, for the periods of June 22, 2001 through July 1, 2001, July 28, 2001 through August 12, 2001 and September 7, 2001 through September 23, 2001. Payment will be made bi-weekly in arrears upon receipt of a properly prepared invoice at the office designated to receive the invoice (FAR 52.212-4(g)). Regular hours (8 a.m. to 4:30 p.m.) are for the following dates: 6-22-01 (8 hrs); 6-25-01 through 6-29-01 (40 hrs); 7-30-01 through 8-3-01 (40 hrs); 8-6-01 through 8-10-01 (40 hrs); 9-10-01 through 9-14-01 (40 hrs); and 9-17-01 through 9-21-01 (40 hrs). On call hours are for the following date and times: 4:30 p.m. on 6-22-01 thru 6:00 a.m. on 6-25-01 (61.5 hrs); 4:30 p.m. on 6-29-01 thru 12 Noon on 7-1-01 (43.5 hrs); 12 Midnight on 7-28-01 thru 6:00 a.m. on 7-30-01 (54 hrs); 4:30 p.m. on 8-3-01 thru 6:00 a.m. on 8-6-01 (61.5 hrs); 4:30 p.m. on 8-10-01 thru 12 Noon on 8-12-01 (36 hrs); 4:30 p.m. on 9-7-01 thru 6:00 a.m. on 9-10-01 (61.5 hrs); 4:30 p.m. on 9-14-01 thru 6:00 a.m. 9-17-01 (61.5 hrs); 4:30 p.m. on 9-21-01 thru 12 Noon on 9-23-01 (43.5 hrs). ON CALL HOURS: When scheduled for on call services, the nuclear medicine physician must be available for duty at the Overton Brooks VA Medical Center within 30 minutes after notification. DESCRIPTION OF DUTIES OF PHYSICIAN: The contractor shall provide the following services: a. Provide consultative services for Nuclear Medicine diagnostic imaging and non-imaging studies. b. Determine appropriateness for diagnostic studies and PET scans. c. Interpret the results of uptake, dilution and excretion studies, including thyroid uptake, plasma volume, red cell volume, dual isotope Vitamin B12 absorption and urea breath test. d. Interpret the results of Nuclear Medicine imaging and localization studies, including thyroid, liver-spleen, hepatobiliary, gastric emptying, acute GI bleed, Meckels, bone, three phase bone, acute venous thrombosis, cardiac blood pool (MUGA), Xe-133 ventilation, lung perfusion, SPECT brain, renal scan with renogram, lasix renal, Captopril renal, tumor and inflammation (In-111 oxine and Tc-99m HM-PAO). e. Interpret the results of Nuclear Cardiology studies including myocardial perfusion imaging using Tc-99m Sestamibi for gated SPECT, rest and exercise and pharmacological stress using adenosine and dobutamine, wall motion and ejection fraction. f. Provide radiation safety supervision and consultative services. CREDENTIALS: For work being performed by the contractor, the physician performing the services shall be certified in: a. Nuclear Medicine by the American Board of Nuclear Medicine (ABNM); or Diagnostic Radiology with a special competency in Nuclear Medicine (ABR); or Diagnostic Radiology (ABR) and currently (or within 5 years) named as an authorized user on a U.S. Nuclear Regulatory Commission or Agreement State Byproduct Material license for uptake, dilution, and excretion studies and imaging and localization studies. b. Average number of procedures to interpret per week, is 40 for Nuclear Medicine and 32 for Nuclear Cardiology. c. Average number of imaging procedures performed on call is one per weekend (usually ventilation and perfusion lung or acute GI bleed). d. The credentials must be submitted with the solicitation. Three types of imaging systems in the Nuclear Medicine Department are Hitachi SMV T-22 Dual Detector, ADAC Forte Dual Detector and Picker Prism 3000 Triple Head Detector. SERVICES: a. The service specified in this contract may be changed by written modification prepared by the VA Contracting Officer and, prior to becoming effective. b. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. c. The services to be performed by the contractor will be under the direction of: Chief of Staff, Overton Brooks VA Medical Center, Shreveport, LA. QUALIFICATIONS: a. Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. b. Personnel assigned under this contract shall be certified in accordance with the terms of this contract (See Credentials). c. Personnel assigned by the contractor will be responsible for meeting appropriate JCAHO requirements regarding quality assurance, such as maintaining current credentials and clinical privileges, peer review, participating in criteria development and date analysis and providing guidance in meeting requirements for nuclear medicine services. d. Every nuclear medicine physician will apply for and be granted medical staff privileges at the Overton Brooks VA Medical Center before being assigned to the Overton Brooks VA Medical Center by the contractor. PERSONNEL POLICY: The contractor shall be responsible for providing: worker's compensation, professional liability insurance, health examinations, income tax withholding and social security payments. The parties agree that the contractor shall not be considered a VA employee for any purpose. PAYMENT: All payments made under this contract will be made bi-weekly in arrears. No advance payments will be authorized. RECORD KEEPING: The VA (Nuclear Medicine Dept.) shall maintain the following records. The COTR is responsible for verifying contractor compliance. a. A sign-in log located in the service requiring the nuclear medicine physician to sign-in and out each day at the time of arrival and at the time of departure. b. Weekly Time Sheets showing the hours each contractor employee worked. c. Copies of the work schedules. d. Master lists of the regular work assignments for each contract employee. e. The Government may evaluate the quality of professional and administrative services provided; but retains no control over the medical, professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment), in accordance with FAR 37.401(b).The provisions at FAR 52.212-1, Instructions of Offeror -- Commercial Items (Oct 2000) applies to this acquisition. Quotes must be in writing and accompanied by a completed Representations and Certifications or they will be rejected. Facsimile quotes will be accepted; oral quotes will not be accepted. Award will be made to the offeror whose quote conforms to the requirements of the solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered.
