Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation is issued as a Request for Quotation (RFQ) Solicitation Number F64605-00-QP093. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. (iv) This acquisition is 100% set-aside for small business. The Standard Industrial Classification Code is 5091 with a small business size standard of 100. The associated NAICS code is 421910. (v) This synopsis/solicitation contains one contract line item number (CLIN) CLIN 0001: Price will be submitted as follows: *CLIN 0001, Monark Ergomedic Cycle, Monark Regomedic P/N 839-E. No substitutions will be accepted, 18 each X $______(Unite Price) = $ _________ (Total Price). (vi) Must include Microsoft Windows operating system and be compatible with Polar Heart Rate monitor, Polar Pacer Item Number 1902061. Acceptance will be made by one point of contact at each destination. FOB point is destination point. (viii) The provision at FAR 52.212-1, Instructions to Offerors _ Commercial Items, applies to this acquisition without addenda. (ix) A firm fixed price purchase order will be issued based upon the proposal determined to provide the best value based upon delivery and price. (x) Offerors are required to complete a copy of the provision at FAR 52.212-3, Offeror Representations and Certifications _Commercial Items, excluding paragraph (b) of the provision (IAW DFARS 212.301(b)(2)). (xi) The clause at FAR 52.212-4, Contract Terms and Conditions- Commercial Items, applies to this acquisition without addenda. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders _ Commercial Items, applies to this acquisition.
