Description

The Connecticut Army National Guard seeks a contract physician to provide the following: smoking cessation, health fairs, immunization, initial employee screenings and medical testing. A fixed-price commercial contract with Best Value Selection (BVS) is anticipated. The performance period will be for one (1) year with four (4) one-year option periods. The Standard Sic Code (SIC) is 8099. The North American Industry Classification code (NAIC) is 621498. This procurement is being set-aside for small businesses. Large businesses need not respond. Responses must include the name and street address of the business, fax and telephone number. Proposal packages may be obtain from the above address in writing. Verbal requests will NOT be honored.
