Description

This is a synopsis for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. Commercial item procedures will be used. A solicitation will be issued. Request for Quotations (RFQ) 623-10-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97- 23. The North American Industry Classification System (NAICS) code is 624310 and the small business size standard is 5 million dollars. The VA Medical Center, located at 1011 Honor Heights Drive, Muskogee, Oklahoma 74401, has a requirement for Chapter 36 Vocational Rehabilitative Services, specifically Counseling Services. The contractor is to perform initial evaluations and appraisals, assess interest, aptitudes and abilities, provide testing and explore vocational and educational goals or needs with the veteran. Evaluations and vocational/educational plans of action will be submitted the VA Regional office Muskogee, Oklahoma. Services are to be performed at the contractor's place of business. Any company performing services as a result of this solicitation is to be physically located within the state of Oklahoma. The government anticipates award of multiple Basic Ordering Agreements (BOA) for these services. Quotations shall be submitted on an "all or none" basis and in accordance with the terms and conditions of the solicitation. No partial quotations shall be submitted. Requests for solicitations shall be submitted to the address listed herein or faxed to 918-680-3852, and directed to the attention of Mick Dubuis (90C). Requests shall be on company letterhead and shall reference the solicitation number (RFQ number) on the outside of your request for identification. Issue date for solicitation is April 2, 2001. The government is not responsible for the quality of faxed requests.
