Description
The Portland, OR Veterans Affairs Medical Center (PVAMC) is seeking a health care agency(ies) to participate in a one year study (Project at Home) that involves providing acute care services at the home of patients diagnosed with COPD, CHF, community acquired pneumonia, or cellulitis. These patients would otherwise qualify for hospital admission but will have been evaluated and deemed safe to recover with care at home. Agency participation would include, as a minimum: Management Participation in developing the: program of nursing support for acutely ill [older] people >65 years old coordination of assets between PVAMC and contractor scheduling and notification requirements. Providing RN, LPN, and CNA services as required for patients who are acutely ill [care.] Providing or subcontracting for Durable Medical Equipment (beds, wheelchairs, etc.) on a limited short term basis. Selected diagnostic procedures (X-ray, EKG, Ultrasound arterial blood gases, etc.) Follow-up patient visits at the patient's home. Data recording, input and evaluation. Providing feedback related to satisfaction with providing at home hospital care Initial patient contact is expected to occur either at the patient's home or in the PVAMC Emergency room after diagnosis, evaluation, and admission into the study. Agency personnel would coordinate with PVAMC medical staff, meet the patient in the Emergency room or at his/her home after release from the Emergency care Unit (ECU), and receive some initial PVAMC provided medications and supplies. At that point the Agency personnel would be expected to assess the patient and home situation; communicate the patient's additional needs to the VA Project at Home coordinator or Project at Home physician; and if required and approved, coordinate the delivery of Oxygen, Home Infusion Equipment and Services; and durable medical equipment. After the initial assessment, Agency personnel would provide patient care on an as needed basis. There will be a minimum of 8 hours of continuous RN care after arriving home. Nursing care after the initial 8 hours will likely be on an intermittent, scheduled basis. Agency management involvement in the planning process would begin as soon as the contract (s) is/are awarded with the patient care portion of the project beginning 1 October 2001 and ending 30 September 2002. Dedicated funding of between $200,000.00 and $300,000.00 for direct patient care costs is currently available for this project. It is anticipated that 100-120 patients will be enrolled with an average length of stay of 2-3 days. This will be a negotiated procurement where Technical Factors are approximately equal to price. If a suitable Agency(s) can be found, PVAMC intends to award a fixed price contract(s) for these services. The contract(s) would start before 1 October 2001 and end on 30 September 2002; on the exhaustion of study funding, or on enrollment of 120 patients which ever comes first. All interested parties are invited to submit proposals. The estimated release date of the Request for Proposals is 30 March 2001, with an estimated proposal due date of 4 May 2001.
