Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-20 and VAAR #20. RFP 621-14-01 should be referenced on all proposals. The NAICS Code for this procurement is 621111 and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Medical Center intends to solicit proposals and award a firm fixed price contract under the authority of Title 38 USC Section 8153, pending availability of funds. The James H. Quillen VA Medical Center, Mountain Home, Tennessee has an EMERGENT need for a Board Certified Locum Tenens Radiologist. The Radiologist shall provide coverage 40 hours per week Monday through Friday, 8:00 a.m. -- 4:30 p.m., excluding Federal Holidays for a period of 6 months with option to extend for an additional 6 months. Scheduled On-Call will be required every fourth week, taking call by beeper and teleradiology equipment Monday through Friday 5:00p.m. to 8:00 a.m. and Saturday and Sunday Noon to 8:00 a.m. (Contractor provided Weekend on-call services includes reading Plane Films and emergency CT's in hospital 8:00 a.m. to noon Saturday and Sunday). Unit Prices quoted shall be all-inclusive of all costs, such as, Transportation, Housing, credentialing/privileging and malpractice. No payment will be made for the scheduled half-hour lunch break. Scope of practice will involve the following services: (a) Normal Tour of Duty shall include Plane Film, CT, US, MRI and routine Barium Fluoroscopy procedures and interpretation, as well as, appropriate interaction with Department Staff and Clinical Staff; (b) No procedural studies to be done after normal duty hours; (c) No Angio/Interventional expectations; (d) Contract physician may at his/her discretion come into the hospital to look at emergency CT's if he/she is not at home when paged and in-hospital viewing would expedite reporting. Radiologist must be proficient in utilizing CPRS electronic medical record systems. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, and income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. Offerors responding to this announcement shall submit a cost proposal and technical proposal to include the following evaluation factors: Candidates experience and personnel qualifications to include evidence of current licensure and credentials/privileges and current CV of candidate, Indemnification and Medical Liability Insurance certificate, past performance of contracts of similar type, scope size and complexity that are ongoing or completed within past three years, references with points of contacts for contracts identified. Offers that do not include CV's of Candidate presented WILL NOT be considered. Offerors may propose more than one candidate. CANDIDATE MUST BE AVAILABLE FOR IMMEDIATE DUTY-PERIOD OF PERFORMANCE TO BEGIN NOT LATER THAN MARCH 26, 2001. The Cost Proposal shall include: (1) Hourly Rate for Normal Business hours $_______. (2) On-Call Weekday/Per Night Rate $_______. (3) On-Call Weekend/Per Day Rate $______. (4) Overtime Rate $_______. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. All documentation required to credential and privilege the physician must be provided to the Credentialing Office at this Medical Center prior to contract award. OIG List of Excluded Individuals/Entities applies. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting officer.
