The Government intends to negotiate a firm fixed contract on a sole source basis under the authority of FAR 6.302 with Natus Medical, Inc. 1501 Industrial Road, San Carlos, CA 94070 for monthly drop shipments of infant hearing screening kits as required by Naval Medical Center, Portsmouth, Virginia. The resulting contract will be from 15 March 2001 to 14 March 2002 with priced option provisions to extend the term for four additional one-year options in addition to the provisions of FAR 217-8 "Option To Extend Services.". This contract will be solicited in accordance with FAR Subpart 13.5, Clinger-Cohen, Commercial Item Solicitation. Potential Offerors are hereby notified that the solicitation will ONLY be available by downloading the documents at http://neco.navy.mil. This office will no longer issue hard-copy solicitations. By submitting a proposal, the vendor will be self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any Federal Agency. Failure to do so may represent grounds for refusing to accept the proposal. Number Note 22 applies.
