- This is a combined synopsis/solicitation #RFQ-618-108-LK-01 entitled "EMG Machine, 4-Channel Clinical." It is prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 97-23 and VAAR 97-8. The VA will conduct this procurement according to a combination of FAR Parts 12 and 13. The North American Industry Classification (NAICS) Code that applies is 339112 and the small business size standard is 500 employees. This CBD announcement constitutes the ONLY solicitation/bid document that will be issued. A separate written solicitation/bid document will not be issued. Contractors are requested to submit their written technical and cost proposals in response to the requirements listed below. Contractor may submit his proposal on his company letterhead or formal quotation form with authorized signature. Contract must address ALL requirements listed in a separate technical proposal explaining how items he is proposing meet the VA's requirements. The Contractor's price and technical proposal must contain the name; title and signature of a person authorized to submit proposals/bids on behalf of the contractor. Descriptive literature for equipment offered must also be included in proposal/bid package. The VA's requirements include One each 4-Channel Clinical EMG Apparatus with the following minimum capabilities and accessories: 400 MHz Pentium CPU w/64 MB of RAM, 6.4 GB Hard Disk Drive, 3.5" Floppy Disk Drive, CD-Rom Drive, 24x Speed, PC LAN Adapter, 17" Color Monitor (1280 x 1024), Color Printer, Trolley with a shelf, 3-Key Foot Switch (including holder) and Control Panel with Mouse.MS; 4-Channel EMG/EP Amplifier Box including Arm; Dual Constant Current Stimulator with 4 outputs, 2 meter; Active Stimulation Handgrip; Quantitative EMG with Spontaneous Activity, Multi MUP, Single MUP and Interference Pattern Analysis; Nerve Conduction to include Motor Nerve Conduction, Sensory Nerve conduction, F-Wave, Inching; Decrement Test; H-Reflex; CV General; Blink Reflex, WinReport Windows Report Generator; Somatosensory Evoked Potentials; Single Fiber; Oscilloscope; Macro EMG; Sympathetic Skin Response; Temperature Measure Inclusive Sensor (31L10); PC-Local Area Network Board Novell 2000, User's Manual. Contractor is also requested to address Length of Warranty, Time of Delivery after Receipt of Order, Installation and Training that will be provided and cost (if any), FOB point and Shipping Charges (if applicable), and Prompt Payment Terms. The Minneapolis VAMC also has a DANTEC EMG machine Model MK2 Counterpoint that it would like to trade-in for credit. Please indicate if your company is willing to offer a trade-in credit. This will be a Firm Fixed Price Contract. Evaluation of proposals offered will include the following factors: The capability of equipment offered to meet the governments requirements; reputation in the trade and past performance history of contractor; and price. Award will be made to the offeror determined to offer the "best value" to the Government. Evaluation may be made with or without discussions with the offerors. All responsible sources that can meet the requirements and provide the items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead or bid form. The following provisions and clauses shall apply to this solicitation: 52.252 Clauses Incorporated by Reference.
