Description
THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. MOD128.01 is being issued as a request for quotes (RFQ). RFQ MOD128.01 and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC)97-23. The associated NAICS code is 325413 and the small business size standard is 500 employees. The Centers for Disease Control & Prevention/National Center For HIV/AIDS, STD/TB Prevention (NCHSTP), requires the following HIV rapid test kits: 1)205,000 EA HEMASTRIP HIV TEST made by Saliva Diagnostic Systems, Inc.; 3661 Horseblock Rd.; Suite E, Medford,, NY 11763; 2)220,000 EA TRINITY UNIGOLD RHIV 20T(PIE) PLC S/N 1206502 made by Trinity Biotech; IDA Business Park; Bray, CI Wicklow, Ireland; and 3)118,000 KITS of ORAQUICK HIV TEST KITS by Orasure Technologies, Inc.; 8505 SW Creekside Place; Beaverton, OR 97008. CDC believes that the above listed companies are the only ones that meet the following requirements: a) Proven sensitivity and specificity in the Malawi AIDS Counseling Resource Organization (MACRO) setting; b) Broad sensitivity to HIV-1 and HIV-2 strains; c) Long shelf life and no cold chain needed; d) Inclusion in an established proven testing algorithm for voluntary counseling and testing; and e) The only whole blood test kit algorithm approved by the Ministry of Health for use in this setting. As a result, CDC intends to make multiple awards to these companies on the basis of sole source. However, all responsible sources that can satisfy with their products the above requirements may submit a quote, which will be considered by the agency. They shall also submit product literature and product sample (failure to submit product literature and sample may rule the quote unacceptable). Delivery of all items shall be made within one weeks after receipt of order, FOB Destination, Within Consignee's Premises to: CDC Warehouse, C/O Steven Sloan, 4998-C South Royal Atlanta Drive, Tucker, GA 30084-3005.
