Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal (RFQ) 623-09-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-22. The North American Industry Classification Code is 621111 and the small business size standard is 5 million dollars. The Muskogee VAMC, located at 1011 Honor Heights Drive, Muskogee, OK 74401, has a requirement for pathology services. Contractor shall provide Pathology coverage for the VAMC Muskogee, OK. There will be a board certified Pathologist (anatomical & clinical) available to provide services as described herein. There may be any number of different individual Pathologists providing this coverage as long as they all are participating in a group practice of which they all belong and have current credentials at the VA Medical Center (offerors not yet credentialed at this facility must possess the qualifications to be credentialed at this facility. Persons not yet credentialed at this facility and who have not yet submitted their Curriculum Vitae (CV) are required to submit a CV at their earliest convenience, but not later than the due date of offers. (All medical professionals must be credentialed in order to work for this facility). When the VAMC Pathologist is unavailable, the contracting Pathologists will provide coverage, as needed during the entire period 24 hours per day 7 days per week. This will be accomplished by being available by telephone in addition to the hours actually spent at the Medical Center. The VAMC Pathologist will be unavailable approximately six weeks per year. When the VAMC Pathologist is on Station, the contractor shall be available approximately 2-4 hours per normal duty day between 11:00 AM and 3:00 PM. Normal duty days are Monday through Friday, excluding federal holidays. Services will be required at the Medical Center in person. Autopsies are normally started between 09:30 AM-10:30 AM. Thus the time of coverage, the hours needed and the number of trips to the VAMC on any given day may vary depending on the specific workload and needs of the service. If needed per the discretion of either party, it is acceptable to provide some coverage at a local facility outside the VAMC if the facility is within a distance of 15 minutes of the VAMC (for the Purpose of transporting frozen section specimens, which must be finished within 20-30 minutes). Autopsies will be performed at the VAMC Morgue. This type of coverage would include but not be limited to reading and interpretations of surgical pathology slides, bone marrows, crystal identification, Fine Needle Aspirates and frozen sections performed at the VAMC or taken to the local laboratory for rapid interpretation as specified above. Access to the VAMC VISTA computer will be made available in order to sign out reports and access patient information. This may be accomplished by a modem from a remote location. Surgical slides may be delivered to a local facility within 15 minutes when necessary and normally there would not be a need to be on site to provide this service. Duties will include surgical pathology gross and microscopic examinations and all attendant functions including coding and report verification. The surgical reports are coded for CPT, VAMC tissue committee, SNOMED and ICD-9 components. Interpretations will occur as per routine surgical pathology practice. Other duties will include providing autopsy services. Cytology reports will be reviewed and signed with provider notification as needed. Consulting with Medical Center Staff and Laboratory Staff on an as needed basis, and performing autopsy QA are examples of other common duties. Estimated Procedure Annual Workload Estimates are as follows: Surgicals -- -- -- 1840; Cytologies -- -- - 610; Autopsies -- -- -- -24; LMIP units -- 701,000. Qualifications: Physicians performing work on this contract must be certified in clinical and anatomical pathology. Physicians must be licensed in a state, territory, or commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the Chief, Pathology Service, the credentialing team, and the approval by the VA Facility Director. Physicians performing work on this contract must be U.S. citizens and credentialed and privileged in accordance with VA regulation as set forth in VA handbook 1100.19, during the duration of the employment. The credentialing and privileging process takes a minimum of four to six weeks. The contractor is required to submit the required information for completion of the credentialing process. Quotations shall be submitted free of special stipulations. Partial quotations shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
