Description

The Contractor will provide all labor, tools, equipment, supplies, materials, and incidentals necessary to accomplish annual medical examinations and special medical screening of personnel at the Defense National Stockpile Center, New Haven, IN Depot located at 15411 Dawkins Road, New Haven, IN. Employees of the New Haven, IN Depot will be scheduled to come to the Contractor's place of business for the laboratory tests and physical examination to be performed. The annual laboratory tests and physical shall include: blood and urine (CBC and Chem Screen), Audiometry (Baseline), Chest X-Ray Two-View once every five years (B reader and PA), electro cardiogram (routine w/tracing, interpretation and report), spirometry/PFT, vision screening, general physical examination, history (occupational and medical), asbestos questionnaire (initial), respirator questionnaire. Special laboratory testing may be necessary to screen for the presence of heavy metal, lead, mercury traces, provide influenza virus vaccination, lyme disease vaccine, and perform a lipid profile. The Defense National Stockpile Center contemplates making an award to one responsible small business contractor for a firm-fixed price requirements contract for one base year with four one year options.
