Description

This is a combined synposis/solicitation in accordance with the format in FAR 12.6, and FAR 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The Request For Quotation (RFQ) number is 539-01-14. The performance period is from date of award through September 30, 2001, with four on-year options to renew. A firm fixed price contract will be awarded. The NAICS Code for this service is 811212. The size standard is $18.0 million in annual sales. This solicitation incorporates the provisions and clauses in effect through Federal Acquisition Circular 97-21. This procurement is unrestricted but is potentially reserved for HUBZone 8a, 8a, HUBZone Small Business, Very Small Business and Small Business participation. The contract will be awarded in the cascading order above in order to provide maximum practical opportunities for small and small disadvantaged businesses. All responses from responsible sources will be considered. In accordance with Federal Acquisition (FAR) Subpart 19, any award resulting from this solicitation will be made on a competitive basis to an eligible HUBZone 8a concerns, provided that a minimum of 2 competitive offers are received form eligible HUBZone 8a concerns. If a minimum of 2 offers form eligible HUBZone 8a concerns are not received, the award will be made to eligible 8a concerns, and so on through thr cascading order above. If a minimum of 2 competitive offers from qualified small businesses if not received, the RFQ will be competed on a full and open basis among responsible firms. This RFQ is being issued to provide full maintenance and emergency repair services on the following facsimile machines located at the VA Medical Center, 3200 Vine St., Cincinnati, OH 45220 and the VA Ft. Thomas Nursing Home, 1000 S. Ft. Thomas Ave., Ft. Thomas, KY 41075. The contract shall include labor, parts and travel. Supplies including drums and toner shall not be covered. Response time shall be within 48 hours of call for service. The facsimile machines to be covered are: (16) Sharp Model F03450, (34) Sharp Model F03850, (12) Sharp Model F04500, (1) Canon Model L775, (2) Canon Model L790, (1) Canon Model 920, (1) Matsushita Model UF127, (1) Matsushita Model UF128M, (1) Matsushita Model UF7500 and (4) Murata Model M900 fax machines. Please quote the cost per machine, per month for the base period and each option year. The Government does not represent that the same equipment listed above will be on hand at the time of award or during the life of the contract. The Government may change the equipment location, delete, add or replace equipment with the same or similar types/models of equipment. Adjustments to the contract price shall be negotiated when equipment is deleted and not replaced or when additional equipment is added. No adjustments shall be made when the equipment is replaced with the same or similar model.
