See notes #1 and 22. This announcement is for information purposes only and will be made on a sole source basis by COB 2/26/01. This award is a planned solicitation under the simplified acquisition process. The Food and Drug Administration [FDA], the Center for Veterinary Medicine [CVM], intends to award a purchase order for services to Clinical Pharmacy Associations, Inc., Beltsville, MD., based on a Scope of Work as follows: collaborates with a variety of hospital types and sizes in the Nat'l Capital area to provide data on clinical practices and multi-drug resistant Enterococcus faecium infection rates such as: data and/or information on the incidence, prevalence and development of vancomycin resistance Enterococcus faecium [VREF] and Synercid resistant Enterococcus faecium [SREF] in environment of participating hospitals. Data to include but not limited to: patient admissions per unit time for periods prior to approval of Synercid [9/99], length of stay, medications, physician prescriptions, provide medical chart to validate dates in databases; existing collaborative relationship with Nat'l Capitol area hospitals to permit collection of necessary data; must provide data to maintain patient confidentiality, supply plan for data acquisition, reporting, chart review, database and final report. Period of Performance: 3 months from date of award. This procurement will be award in accordance with FAR subpart 13.5 and 52.232-33, 52.227-2, 52.227-14, 52.227-15, 52.227-16, 52.227-17, 52.227-18, 52.227.19, and FDA 1337.
