Description

Primary Care Services in Monroe County, Ms. The VA Medical Center, Memphis, TN is seeking offers from Medical Care Providers for Pirmary Health Care Servies for enrolled veterans. Payment for services will be on a montly capitated rate basis. The contratt term will be one base year with four 1 year options for contract extensions. The primary health care services to be provided will be in Monroe County, Ms. Contractor is expected to meet or exceed JCAHO requirements for Primary Care. Fcilities must be in compliance with the American Disabilities Act requirement. Te VA is expected to enroll approximately 1400 patients per year. The contractor shall provide all personnel, clinic facilities, equiment, supplies, and supervision for the provision of primary health care servies. The contractor will be responsible for the provision of all radiological and laboratory servies asociated with primary health care services. The contractor will be responsible for writing short term pharmacy prescriptions to be filled at a local pharmacy under separate contract. The contractor must have a primary care provider on call 24 hours per day, 365 days a year to respond to enrolled patient telephone inquires. The VAMC will make and award to offer that represents best value to the VAMC. Contract award will be based upon the evaluation of offers using selection factors including cost as one factor.
