Description

This is a combined synopsis/Request for Proposal (RFP) for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Responses to this notice should be provided in writing by 16 February, 4:00 PM PST. This is an Indefinite Delivery Indefinite Quantity(IDIQ) type contract, with one base year and four option years. The solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 97-21. The Government intends to procure the following: A continuation of a preventative health and wellness program to promote physical and mental health fitness for both military and civilian employees of the American Forces Information Services (AFIS) located at 601 N. Fairfax Street, Alexandria, VA 22314. This Performance Work Statement (PWS) will include membership benefits and responsibilities, wellness program participation and reporting requirements. This facility should be available within a 5 minute walking distance from the above address. The PWS and line items can be accessed on the following site: ftp://www.tasa.army.mil/pub/1L8. All responsible sources may submit a quote, which shall be considered by the agency. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source, Old Town Sports and Health, Alexandria VA 22314, under the authority of FAR 6.302-1. This notice is not a request for competitive proposals. However, all proposals received by the time established for the receipt of proposals will be considered by the Government. The applicable NAICS code is 713940.
