Description

Contractor shall provide one (1) Locum Tenens Anesthesiologist to provide services at the VA Medical Center, 4150 Clement Street, San Francisco, CA 94121. The Contract is for three month periods which includes a base period and four (4) options to renew. The Anesthesiologist is to provide services until a permanent position is filled. This is a commercial procurement. The following clauses and any addendum apply: 52.212-1 Instructions to Offeror -- Commercial Items, 52.212-2 Evaluation -- Commercial Items, 52.212-3 Representations and Certifications -- Commerical Items, 52.212-4 Contract Terms and Conditions -- Commercial Items, and 52.212-5 Contract Terms and Conditions to Implement Statutes and Executive Orders -- Commercial Items.
